What Causes Severe Pain In Rectum Area, Left Buttock And Blood In Stool?

Question: My daughter, who is 18, is having severe discomfort in the area of her rectum, but only on the left side. She said it feels like her stool is directed down at an angle, not like it would pass straight out her anus, but rather like it is aimed off towards the left side of it and is running into her left buttock. Another way she described it is like only half of her anus would allow a stool to pass through - that the other half feels cut off, squeezed, or blocked. Last Wednesday (almost a week ago), she had a significant amount of blood come out when she had a bowel movement. There was no strain as there is when she is sometimes constipated, and the stool was relatively soft, but there was a significant amount of bright red blood floating in the toilet and on the toilet paper when she wiped. We are an eight hour drive from home and will be driving home tomorrow. Just wanted to check to see if you had any suggestions as to what it might be or what steps to take...

Thanks for your query related your daughter who is 18 years old.
My thoughts after reading the history you have provided:
Since she has the feeling that there is something on the left side like cut off, squeezed or blocked and had bleeding per rectum last Wednesday and feels like the stool is directed down at an angle towards the left side, there has to be something.
This probably may be a thrombosed hemorrhoid on the left side causing all the symptoms. Another possibility is development of a peri-anal abscess on the left side.

I would advise her the following since you are an eight hour drive from home and will be driving home tomorrow:
- Laxatives to keep the stool soft.
- Rest in lying down position (and preferably the leg end of the bed elevated).
- To take an anti-inflammatory medicine with a painkiller property like Ibuprofen if it suits her in the past also.
- Sitz bath an a tub with warm water, particularly before and after passing stool.
- She needs to be seen by a Doctor, preferably a General Surgeon and if possible to visit the local ER so that appropriate antibiotics and other medicines may be started if required and she has then a comfortable journey back.
- Timely treatment helps to avoid further complications.
- Full normal diet, avoid only the foods and beverages that she might have noticed in the past to cause constipation.
