Detailed Answer:
dear,
Welgome to HCM
As I understand, your major compliant is of chest pain, nose clogging, bleeding of nose on removal of clogs, background of chronic smoking hookah and cigarettes.

Chest pain can have varied etiology, such as respiratory causes of pneumonia, pulmonary infarction, etc cardiac causes of ischemia, etc.
However one needs to consider associated symptoms of breathlessness, cough, edema on legs in these conditions. The nature of pain needs to be evaluated further.
May I have answers to following questions.
1) what is the nature of your chest pain, sharp shooting, radiating to arms, dull?
2) breathlessness, dizziness, swelling over legs?
3) cough, expectoration, fever?

It will help us to differentiate between respiratory and cardiac condition if present.
Kindly describe them interns of duration and its manner of onset.

For your chest pain
1) get an urgent ECG, chest XRay done.
2) do consult local physician soon.
3) tab voveron 75 mg can be taken as and when required XXXXXXX thrice a day, till you consult a physician.

Regarding nose clogging,
I would suggest use of otrivin Nasal drops to clean your nose clogs.
Do not be harsh on nose blowing.
Maintain adequate hydration, overuse of antihistaminic over the counter medication can produce dryness of the nasal mucosa that can lead to frequent bleeds. Proper nasal hygeine can solve this problem.
An indication of antibiotics can be possible in case of respiratory infection, Therfore a physician consultation is required as you can't obtain antibiotics with out prescription.

Hope to have satisfied your query.
Welcome for further suggestions.

Brief Answer:
Muscle sprain

Detailed Answer:
thanks for the reply.
Chest pain only on movent like turning and bending, is more suggestive of due to muscle sprain or spasms.
one need to evaluate the compliants of swelling through proper palpatory examination, that can be done by physician.
my advice would be,
1) get a chest x Ray chest done.
2) get an ecg done.
these two investigations must rule out any grave pathology.
Considering background of chronic smoking there are very much advisable.
3) continue tab voveron 75 mg as advised before.
4) you can follow my advise for nasal hygeine given before.
5) do consult a local physician

welcome for futher suggestions.
