Question: I am 72 years old female and for the last three days I have trouble catching my breath. I also have a pace maker put in a few years ago. It seems to be getting worse. I couldn't sleep last few nights. should I be going to an emergency room. even walking is difficult without stopping.

I read your query and understood your concerns.

You have experienced dyspnea (trouble catching your breath) over the last three days that aren't relieved by pulmonary bronchial inhalers.

All your symptoms are typical for patients with heart failure. Moreover, the fact that your symptoms are getting worse even on minimal physical efforts shows that you may have a severe heart failure(HF).

This is why I strongly recommend, you should go immediately to ER, in order to confirm the diagnosis of HF and to start the appropriate treatment.

If you were my patient I would have done on ER settings the following examinations:
- Blood work including pro BNP levels which confirm the diagnosis of HF
- ECG
- Chest x-ray
- Cardiac ECHO

After the first evaluation you should be put under the adequate treatment with beta blockers and water pills to treat the HF and to relieve your symptoms.
