What Causes Redness On Ankles, Swelling In Legs When Creatinine Level Is 180?

default
Posted on Mon, 6 Apr 2015
Question: I have abruptly developed a creatinine level of 180, creatinine clearance of 46,st 2 months ago all of these parameters were within normal range. These readings are accompanied by redness around ankles and swelling of the legs, which may have in part another etiology (golfers vasculitis) since this has occurred once before after too much continuous walking. I am diabetic but well-managed at 6.2 hbA1-c. Can yje creatinine paramters be brought under control, and might they be related to the golfers vasculitis nonetheless?

i also produced 250ml of urine every hour+, even through the night

Brief Answer:
AKI

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.
You have developed Intrinsic acute kidney injury (AKI) consequent to vasculitis.

Excessive walking also can cause Rhabdomyolysis .
You need further work-up .

Brief Answer:
You need further investigations

Detailed Answer:
Hi,

To exclude Rhabdomyolysis as the underlying cause of acute kidney injury (AKI), Serum myoglobin, creatine kinase levels ; urinalysis (to look for heme positivity) are to be undertaken.

If the tests are negative, then tests of vasculitis causing acute kidney injury (AKI), such as Serum ANA, XXXXXXX AGBM antibody, hepatitis serology, cryoglobulins, complement levels have to be undertaken.

It is better you consult a nephrologist, since your creatinine level is very high 180 (normal range 44–80 mol/L) and your GFR/creatinine clearance is 46 (Stage 3 renal insufficiency =30–59 mL/min per 1.73 m2).
