Hello and Welcome

I appreciate your concern
Based on your description, its likely to be an infective or allergic inflammatory cause. Antibiotics like amoxicillin plus Clavulanate have good anaerobic coverage too and are usually prescribed for infections associated with the anal region.

The area needs to be examined for the possibility of an abscess or fissure or hemorrhoids, preferably by a General surgeon or proctologist. For the time being OTC posterisone forte or metronidazole gel and xylocaine gel can be applied for relief and treatment purpose.

Wishing you best of health

Thanks
