Question: Hi,
I am 31 year ols male patient with query for experienced Opthalmologist
In January 2014, i consulted opthalmologist in vasan care XXXXXXX complaining redness in eye and with conjunctivitis like symtoms (i had severe throat infection followed by severe redness and itchy eyes).
Doctor confirmed it as viral conjunctivitis and prescribed vigamox (brand alcon) and tears natural forte eye drops.
The redness and itching reduced after 2-3 weeks in my right eye but it did not subside in my left eye.Infection /Redness recurrant and was feeling severe pain in my left eye and was photophobic.
Doctor prescribed FML-T drops and this i continued for 1-1.5 months.Doctor had suggested to gradually reduce the dosage from 4 times per day and then 3, 2 and finally 1 time per day.But after i stopped the drop usage, the infection and redness recurred in my left eye and then finally ended up consulting a cornea specialist
.
Currently i am continuing with Prednisolone acetate opthalmic drops and refresh tears from allergan for past 4 months due to recurrant redness in left eye every two weeks or so.
It reduces once the doasage is 3-4 times per day but as soon as i reduce it to 1-2 times per day(as per the advise from doctor to gradually reduce the dosage), the infection in cornea comes back and there is severe pain and am unable to stare at light.

All the doctors i consulted are unable to give me a reason for this problem and it is not helping me in anyway in reducing the worry.

Kindly request you to advise me on what could be the reason for this and the options available with me to prevent/cure this.

Brief Answer:
Antiviral topicals will offer excellent cure

Detailed Answer:
Hi, sir
I have read your query and understand your concern.

Going by your clinical history you seem to have "relapsing form of kerato conjunctivitis" secondary to the viral sore throat you had earlier to this.

You seem to respond to only high doses of steroids.Currently you are only placed antiinflammatory therapy as the main stay of treatment, but not prescribed antiinfective therapy.

I strongly recommend to you start topical antiviral therapy like Cidofovir for 3-6 weeks, which can produce excellent remission of your condition in left eye.

Topical cycloplegics like Homatropine can reduce the glare and photophobia when used along with above.

These drugs should be taken only under strict medical supervision.

Frequent cold compress helps in reducing the eye pain and inflammation.

I hope this info is helpful to you.
Thank you

Hi Doctor,
Thank you for responding to the query.Appreciate if you could also throw light on few more of the queries that i have based on your response.

1) Why is this confined to only left eye ?

2)What are the reasons that aggravate the condition? Is stress a factor too?

3) Is it not completely curable - The condition has been recurrant in my left eye for past 7-8 months (once in every 2-3 weeks or so)

4) Is this contagious ? And if yes, what are the feasible precautions that i could take to prevent it from passing on to my close family. The reason why i am asking is because ,as a parent, i am wary about passing this to my 1 year old kid

5) Is the Kerato Conjunctivitis same as Eye Herpes / keratouveitis ?

Brief Answer:
Disease is largely contagious.

Detailed Answer:
Hi sir,
Welcome back.

1.The left eye is more frequently involved due to anatomic inclinations of the left nasonalacrimal duct favouring the spread of infection from the throat to nose to left eye .

2.Dry weather, dust, dry eye, physical stress and excess near work can aggravate the disease.

3.The disease is completely curable with Cidofovir.
The relapses in your case is due to absence of specific antiviral therapy.

4.Yes the disease is extremely contagious.Use eye shades, have regular hand washing, stay away from cuddling children, isolate your room and belongings if possible.

5.Yes in large proportion of cases this is synonymous with Herpetic Kerato conjunctivitis.

I hope this info is helpful to you.
Thank you
