What Causes Red Pimples On Body?

default
Posted on Tue, 24 Nov 2015
Question: I have uploaded the picture. I am noticing small red ulcerated areas, they show up in different locations in that band. Is this fungal infection ? what is it ?. I applied neosporin for past 4 days. In that band I see few red pimples at three location that have small ulcer, No pain, nothing. I do see some healing in non red areas, but these small red areas pop up, then heal and fade away, new ones form at other location in the band. However the band has healing in some section and skin there is less red

is this fungal infection, if it is, why there is no pain ? or is it just irritation that's lasting longer and taking longer time to heal ?. The new pop of small red pimples, that are bright red first day, then show small ulcer and slowly fade in 3-4 days

Please advise

Brief Answer:
Now its time to start some more potent antiinflammatory agents for you.

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

I have reviewed all the data and uploaded pictures. I appreciate your efforts for the picture.

Well, now we need to start with a more potent anti-inflammatory agent and you need to apply Tacrolimus Ointment. Along with that you need to apply Ciclopirox ointment and keep applying Neosporin as this is a place best suited for bacterial and fungal infections. Humidity and uncircumcised skin irritates the glans skin but it is getting better now and going in the natural way of disease.

Redness will slowly go down. Don't skip the medication doses.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.
For any disease you have, a timely follow-up is very necessary regarding the treatment efficacy and to see if there is a need to change the treatment options. So keep a follow up to see how you doing (Always write down my name in the start of your questions at HCM & I will be with you in no time).

isn't Tacrolimus is used for auto immune disorders ?. Are you saying this is auto immune condition or more like fungal/bacterial infection ?, most autoimmune conditions have red bands. Tacrolimus is rarely used for antifungal, but mostly for autoimmmune.

Ciclopirox is used for fungal, but Tacrolimus is strictly used for auto immune disorders. I am confused now because you are pointing to autoimmune . Auto immune conditions means EQ or Zoon or Lichen planus. I don't see anything like that.

I see this more as fungal and bacterial issue with the red spots, but why there is no pain, I do have little burning pain during urination, very little though, other than that, nothing as such, no pain in glans or anything.

Could you please clarify ?. is this auto immune or fungal infection. The line of treatment will change completely based on that. I don't think these are OTC medications

Thank you

Brief Answer:
Yes you need both of them as they are beneficial for the issue.

Detailed Answer:
Welcome back!
I am here for helping to relieve your doubts.

Well Tacrolimus has many uses and I have prescribed here to cover those pinpoint red lesions which come and go. I was waiting for that if they come, we will then start it. Yes it is also used in some autoimmune diseases too including skin issues and balanitis. We have to go deep into the Pathophysiology od the disease at the moment and it is now needed to stop these eruptions of red color. When inflammation goes long we need to go for antiinflammatory topical substances that may cause a local effect on the inflamed tissues. Now I have to go a bit deep into the concept. To help you get that clear we have to understand first what inflammation actually is???
Inflammation is the body's local response to some irritant or inflammatory substance. If there is undue response of the body to the irritants then we have not only to stop/and/or remove that irritant from the skin (in this case now fungus has come into play) but also we need to stop body from overreacting (through use of tacrolimus).

Use of tacrolimus never means that you have got some serious autoimmune disorder that is incurable and to get feared. We Physicians know better what to use, why and when!!!

Medicine is ever revolving field and needs to change according to the situation. A medicine good today may not be tomorrow for the same patient. A medicine not needed today may be the gold standard drug of choice tomorrow for the same patient.

We have to control your infection that has now been superimposed by fungus and the reason is the presence of humidity in the area and the uncircumcised foreskin also irritates.

Neosporin is to avoid the ulcers from getting bacterial infection.

Pain while urination is no worry. Take cranberry juice for that and it will get resolved.

Yes these are not OTC medicines but you need them for sure.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. I have uploaded the latest picture, please take a look. You can see rest of the RED band is fading away except the 2-3 RED spots left. They are less red today than in the morning. I don't think I can get tacrolimus without prescriptions, it's not OTC.

Are the RED eruption fugal spots ? please write YES or NO in front of is so that I clearly know they are fungal.

Today I applied betnovate-C ointment I had had used in the past that is combination of anti-fungal/bacterial because I do have some groin itching for long time. I use betonovate-c occasionally. I am prone to humidity infection followed by itching. It seems to have helped to clean rest of the area as redness in other area as subsided except those 2-3 red spots.

You can see in the latest picture I have uploaded. I also see wrinkles better and roughness of RED band has reduced and it feels more aligned with the glans skin. I assume lack of sharp red demarcated areas this means there is no zoon/EQ/Lichen planus - I fear all three the most because they are not curable. As of now it appears to be more fungal - please Confirm YES or NO here

Please do not close the thread as it become difficult to show progress and ask for treatment

Thank you and appreciate your help.

It is not Zoon/EQ or Lichen Planus. Don't worry about it.

Welcome back!

I have copied your queries and answering here by pasting them one by one just for your convenience.

Yes- I have reviewed the picture and appreciate the efforts for uploading that. Yes it is an Rx Drug not OTC and definitely you will need a prescription from a local physician for the same.

*Are the RED eruption fungal spots ? please write YES or NO in front of is so that I clearly know they are fungal.

Yes---fungal infection is very likely in this case as this is a place like that. In the start, it was, but now Yes and you may take antifungal Clotrimazole if you are unable to have a prescription for Ciclopirox (although ciclopirox was needed here more than clotrimazole, but no problem we can apply clotrimazole too). OR you may use the Betnovate C cream as you are using already. Let me add here that Betnovate C is also anti inflammatory and immunosuppressive and is also used in Autoimmune skin conditions.

*You can see in the latest picture I have uploaded. I also see wrinkles better and roughness of RED band has reduced and it feels more aligned with the glans skin. I assume lack of sharp red demarcated areas this means there is no zoon/EQ/Lichen planus.

Yes definitely there is no Zoon/EQ/Lichen. And I had cleared you about it in the very start of our interaction at this forum.

* I fear all three the most because they are not curable. As of now it appears to be more fungal - please Confirm YES or NO here

YES...

Hello

I am noticing the red patch is constantly changing. Now it looks little more red with some demarcation. I also noticed little shiny and puffed up appearance. I have attached the image. I wonder if this sort of psoriasis. I doubt this fungal infection because there is NO itching, pain or anything . It started as smooth RED lesion and appears the same. I thought it was fading away, but it's back again.

It's looking more like autoimmune condition to me. I assume it is not Zoon/EQ/Lichen. I wonder if it is INVERSE PSORIASIS, where smooth shiny/red area formed in moist location, well demarcated, but it does not have peppered red color, but typical of inverse psoriasis color. I may be biased, not sure . I do have small area of inverse psoriasis in my one armpit. However it has not spread anywhere and it's been there for years. It itches, so it's not clear why the one on penis is not itching like the one in the Armpit. However I read it tends to come under folds. What do you think ?

I have attached new picture, I was feeling better yesterday thinking it's fading away, however it's RED again. Do you want me to continue applying Betnovate-c or try out something. This problem is very frustrating and taking toll on mind fearing if this is something serious. I don't know if this plain balanitis/psoriasis/candid balanitis/Zoon/EQ. I feel everything is possibility now. It's hard to figure out what's going on.

Please advise

Thank you

Brief Answer:
Please Observe some more patience.

Detailed Answer:
I welcome you back with a follow up. Thanks for uploading the picture as a latest file. It is helpful when compared in the review list with older pictures.

The color change is totally natural and this is how inflammatory conditions go. It is natural course of the disease. The skin conditions need some patience and over-apprehension creates more stress and remember one thing: Stress causes the infections and inflammations to aggravate instead of getting cured.

So, I suggest to bear with the situation and wait for it to get better. If you see in detail to the diseases on internet you will like a "man who was a hospital in himself".

This is a place where bacterial and fungal infections attack much, and there are chances for fungal infection to get more severe. That's why I suggested the treatment options for you yesterday, but you had some issues with prescription drugs. But no problem everything is going OK and in the days to come you will be fine.

No it is not always necessary that fungal infection should present typically in the same way.
No it is not Psoriasis.
No this is not Inverse Psoriasis.
It has no connection with your Armpit lesion. Please don't get confused.

Keep applying Betnovate-C and Triple antibiotic; and Clotrimazole ointments; and if possible to get a script from a Physician locally, you better go for the ointments I suggested yesterday. But if you use the 3 I mentioned now, I am pretty sure there will be betterment but it takes time to cure in either way.

Let me know in a timely fashion if you get any urinary issues.

So you are saying I need to apply all them together ?

1) Betnovate-C
2) Triple antibiotic (Will Neospoirn work ??)
3) Clotrimazole ( is this OTC? if yes I will buy from Wallgreens)

Just want to be sure, this is no Bowen's disease and Zoon ?. That's my biggest worrybecause this RED color and painless red patch is what makes me worry. If there was some pain my mind would have assumed it as Balanitis and I may have even forgotten about that. I can tolerate pain, but this painless thing is worse than actual pain. My oral lichen planus somehow makes me worry about SCC and all auto immune conditions - That's the root cause..

There is superficial ulceration in 2-3 locations - but there is no itching, no pain and nothing - that's what worries me thinking if it is zoon/EQ/lichen type of diseases. As far as I read Balanitis causes pain , discharge and pain - These are totally painless lesions - that's my main cause of worry , nothing else. If baceterial/candida balanitis can have painless symptoms like I am having - It may take all the worry of my shoulder - This painless thing, with very superficial mild ulcerations, oral lichen planus and autoimmune worries is the root cause of worry - Nothing else..

I greatly appreciate your help and deeply thankful for all the things you have done so far. It's been great experience, I will give you 10 star if that is possible..you have been fantastic and patient to deal with my worried mind - indeed

Brief Answer:
Yes you need to apply the three topical ointments.

Detailed Answer:
I welcome you once again here at HCM with a hope that you get healthy soon. I am giving answers in the sequence you asked and I have added all the answers accordingly.

"""""""""So you are saying I need to apply all them together ?
##Yes. Apply all the three.
1) Betnovate-C
2) Triple antibiotic (Will Neosporin work ??)
##Neosporin and Triple antibiotic are one and the same thing. they are not different.
3) Clotrimazole ( is this OTC? if yes I will buy from Walgreens)
##Yes this is available OTC and you may easily get it from any local pharmacy.

Just want to be sure, this is no Bowen's disease and Zoon ?

##No this isn't like that. Please don't worry about that and just wait for the days to come when you will be all OK.

##Yes this is possible to have superficial inflammation on the skin. What we do is, we go for Prophylactic bacterial and fungal cover so that the inflammatory area doesn't get secondarily infected by these pathogens and opportunistic pathogens always for such moments to attack already inflamed areas of skin. That's why I told you in the very beginning that Neosporin we need to keep applying to avoid infection by bacteria. Now I also recommend for antifungals too.

I greatly appreciate your help and deeply thankful for all the things you have done so far. It's been great experience, I will give you 10 star if that is possible..you have been fantastic and patient to deal with my worried mind - indeed"""""
