What Causes Recurrent Lower Back Pain Radiating To The Groin?

Posted on Wed, 29 Jun 2016
Question: I have been having very bad abdominal pain off and on for about 8 months. The pain starts on my lower right side back and comes around to my groin. The pain will calm down after 400 to 800 mg of advil. I have been to an gynecologist and he says I have a small fibroid, and he doesn't think that is the source of the pain. One of my clients (who is a surgeon) tapped on my back when I was having the pain and it hurt but didn't make it worse. I went to the ER at his suggestion, and although they fast tracked my there was still over 20 people ahead of me, so I left... My abdomen is sore now and hot like I did too many sit ups. Could this be an ovarian cyst? is there a test for that?

Brief Answer:
Clinical evaluation and examination, investigations to get a cause

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/47 - abdominal pain - 8 months - starts at right lower side back and comes around to groin - calms down with 400-800 Advil - Gyne says a small fibroid - not a source of pain - Surgeon tapped on back - hurt - but did not make it worse - ER - came back - abdomen sore now - hot like done too many sit-ups - wants to know whether this is an ovarian cyst and whether there is a test for that ....

To answer your queries first:
This may be an Ovarian cyst but less likely as the history does not fit well into this.
Ultrasonography and CT scan of the abdomen can find this out.

My thoughts:
The character of the pain suggests the following possibilities:
- Right ureteric stone and kidney related problems
- Appendicitis with fecoliths
- Rarely Spinal problems.
- Post herpes if there was herpes 8 months before.

I would advise you the following and there is no point in loosing more time as you are already suffering for such a long time.
- Tests of stool, urine and blood.
- Ultrasonography followed by CT scan if needed.
- MRI of the spine if clinical evaluation suggests so.
- Clinical evaluation, actual complete examination by a General Surgeon and to get the clinical and pathological tests reports to get to the cause of this pain and have a plan for proper treatment.

I hope this answer helps you to get an early and proper diagnosis and treatment, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
