Question: Hi Doctor! My test speaks following of some important factors.

Hb - 12.8
FBS - 168
Creatinine - 0.9
Bilirubin - 1.5
SGOT - 64
SGPT - 98

LIPID Profile
--------------------
Total Cholesterol - 211
HDL Cholesterol - 51
LDL Cholesterol - 76
VLDL Cholesterol - 84
T.Cholesterol / HDL Cholesterol Ratio - 4.13
Triglycerides - 420

FibroScan - E (Kpa) - 3.8
Fatty Liver - CAP (dB/m) - 357

To add more details, I m 5'9" and weigh 76 KG. I am weekend traveler and has a bit of work stress as well. Do not exercise daily.

My Questions are:
1. Why Jaundice has happened to me twice in last 13 months and thrice in last 5 years?
2. What precautions and medication should I take to cure this.
3. Would Heptral 400 (BD) is recommended for my kind of report, would it help improve my vital / critical values in the report?
4. What mg of Udilive should I take? and what else ?

Kindly write a detailed prescription and your valuable suggestions.

1. high cholesterol, high sugars and probably some underlying infection may be responsible for liver injury.
2. you must be on insulin, statins and liver protective drugs
3. udiliv 400 mg twice a day or heptral 400 bd may be taken .
i have answered your queries, now you please tell me what are your present medications? are you tested for HAV, HBsAg, HCV ? auto immune hepatitis?

What indicates that my Liver is injured?
I have just got this diagnosed today and yet to start my medicine which is as follow..
1. Heptral 400 (BD)
2. Udiliv 600 mg (BD) - ain't it too high?
3. a Multi vitamin (BD)
4. Fenofibrate (OD)
- for 20 Days

Should I start this medication or wait for some more diagnosis as mentioned by you?
What does HAV, HBsAg, HCV and auto immune hepatitis tests help us know or why are these important?

deranged liver enzymes, high bilirubin.

Dear sir, you have raised liver enzymes, high bilirubin, history of jaundice thrice. all these are pointers of liver insult or liver injury, or what we call hepatitis. now to find out the cause, we do tests as i already told you.
you may start the treatment, these reports will not change with the treatment. your liver profile, enzymes will improve.
above all you must control your blood glucose with insulin as all oral antidiabetic medicines will further harm the liver. regards!

I have got all of these three tests done now, and all are negative. Kindly suggest what is the prescription from your end and in what quantity. How and What do we need to control in a priority sequence for earliest recovery.

Dear sir, Hi! all reports negative means you have fattyliver infiltration with no underlying infection. So you must start with the treatment you have already been suggested. in addition to that you must be on insulin for diabetes. I would start you on novomix 30 twice a day , once before breakfast and once before dinner. The dose of novomix should be calculated depending upon your blood sugars. I might start you on a dose of 24 before breakfast and 18 before dinner. You will need to monitor 4 times a day till we finalize the dose. You can take help of your local doctor for a prescription and treatment monitoring.
