Question: Hi
I have a few sore, 1 on my penis head, and a few at the side. I went to a doctor and he gave me anti biotic cream and my penis glans started to swell so bad it hurts till I cant sleep. I went to the doctor again and he told me to continue the cream, but the problem is after few days, the red spots did not appear to get better but the pain get worst every day. According to the doctor I applied too much and when my foreskin cover it up, the cream becomes too powerful. He said it isnt a STD as the spots are flat and there is no pus coming out

The red spot are still painless and no pus at this moment of time. no pain during urination as well.

One night my penis glans hurts too much and I went to another doctor as I was worried. The doc took a look and told me its herpes. The only symptoms of herpes I had was the swell up penis glans. I was prescribe with acyclovir 800 to be taken every 4hrs and acyclovir cream to be applied. Yesterday was the first time I applied and this morning the red spot got bigger. It still does not have pain/ pus at this point of time. The penis glans seems not so swollen when I didnt apply. Should I continue to apply the cream? Is this herpes?

Did you have history of unprotected sexual intercourse ?
If so, are you aware whether your partner has history of herpes in the past ?

Herpes genitalis usually presents with pain and redness, then grouped clear fluid filled blisters develop, which ruptures to form shallow multiple sores. There will be swelling and pain associated with ulcers and usually it heals spontaneously within one week.
The ideal dose of Acyclovir for herpes genitalis is 400 mg Acyclovir thrice a day for seven days for the first episode and for five days for repeat episodes.

hi I have attached the pictures. the sores are not pain when I touched it. This should be the 3rd week of these sores now and every time I put some prescribe cream, it got bigger and make me paranoid. Should I continue instructions of the intake of acyclovir from my doctor which is 800mg per 4hrs?

Saw the ulcer. It does not look like a herpes ulcer. An ulcer which is painless lasting for 3 weeks should be investigated further. I feel you need to go see a venereologist and do some side lab investigations including dark ground microscopy and tissue smear. You need to do VDRL test and HIV Elisa blood test. Clinically it looks like a syphilitic chancre. Granuloma inguinale is another painless STD, however it does not clinically look like Granuloma inguinale. A close physical examination with palpation of border is required to opine better. That is why I suggest you consult an STD specialist.

Hi. Thanks for the reply. Just a correction, I feel pain at the swollen penis glans. I went to another doc yesterday and he diagnoise it as balanitis. He recommend h2h2 water for cleaning and gave me some antibiotic powder to apply after and ciprofloxacin XXXXXXX anti biotic. I been feeling my bottom right back pain and pain in groin area. On a side note, the ulcer in the penis glans have burst and seems to tear the glans. Should I see medical attention with regards to these 2 situation? ( back and groin pain and the tear) Or will both go away and heal when the condition gets better? Is it normal? Thanks

Groin pain and back pain could be connected to the genital ulcer. Infected wound over penis can be associated with painful inguinal swelling. An STD called Lymphogranuloma venereum (LGV) can have inguinal painful swelling and pelvic pain. That is why I recommend you to see a venereologist. Ciprofloxacin won't work against LGV or Syphilis.
According to syndromic management of genital ulcer you need to be treated for syphilis, LGV and chancroid.
Both conditions will go away with adequate treatment of ulcer.

Hi. Is it possible to be infected with such STD considering that I do not feel any pain during urination for the whole period of time? Will the broken skin in the penis glans heal properly on its own? considering the ulcer broke and the top and bottom part is tangling. Thank you

Yes it is possible.
The organism causing urethritis is different from that causing ulcer. Burning sensation while passing urine is a symptom of urethritis. You need proper treatment for the skin to heal properly. Syphilis might heal on its own, but it will remain in your body and later can present as secondary or tertiary syphilis later. Therefore timely and proper treatment is very important.
