Question: Hi! I'm an active 38 year old. Today I noticed that a vein in my right foot appear extended by my ankle. It does not hurt, just appears puffy.

Hi and welcome to HCM:
In most cases this is normal as reaction to warm environment, longer sitting or standing or after specific physical activity. If there is no tenderness which can be palpated and no pain then I doubt that there is thrombophlebitis. You should avoid prolonged standing, try to walk more, apply cold compresses, elastic socks. and see what happens. if worsens, you should do doppler ultrasound to check vein blood flow.
