What Causes Proteinuria?

Question: I was told by my Cardiologist that 'protein is spilling in to my urine'. What does this mean? What causes this? Should I be concerned? What happens next?

Regarding your concern, I would like to explain that "protein spilling into urine" means proteins existing in circulating blood for certain pathological reasons are leaking from blood into the urine.

The above phenomenon usually doesn't happen, as the kidneys possess an efficacious barrier which filters blood from waste metabolism products but normally doesn't permit the loss of protein.

In certain conditions such as in diabetics or in non-properly controlled hypertensive individuals this barrier partially losses its functionality to some degree and protein passes into the urine (easily detected by a simple complete urine test).

Several reasons may be responsible for proteinuria (abnormal presence of protein in urine) such as:

- diabetes,
- arterial hypertension,
- several chronic kidney diseases,
- amyloidosis,
- glomerulonephritis,
- kidney infections,
- sarcoidosis,
- certain medications,
- toxins,
- multiple myeloma,
- heart failure,
- hematologic disorders

But also some special conditions not related to specific diseases may produce proteinuria, such as:

- strenuous exercise,
- heat exposure,
- emotional stress,
- cold exposure,
- fever

Coming to this point, it is necessary to perform a careful differential diagnosis of the above mentioned alternatives.

You need to discuss with your attending doctor for building a rationale strategy of investigating the exact responsible cause of proteinuria.

As the kidney disease are generally silent at the beginning of their dysfunction, it is necessary to seriously consider this laboratory finding for treating timely and reversing completely the underlying pathological disorder.
