What Causes Post Operative Pain In The Stomach And Shoulder After Hernia Repair?

Question: My husband had surgery last Monday April 28th for gastric sleeve and hernia repair. He had this done in Mexico and the doctor prescribed Ketorolaco (Toradol) for pain but it reacts with his blood pressure meds and they took him off for a few days. After he was released from the hospital, we decided that the pain meds reaction with the high blood pressure meds was too much risk so he has not taken any more of the Ketorolaco. However, he is now having pain and wants some relief. I believe it is from the gas left over after the laporoscopy. We go to a local doctor tomorrow for advice but he is in a lot of pain right now. I gave him Milk of Magnesia for the discomfort of the gas and constipation. What else can I do? Sorry for the long email.

1. Where is the exact location of pain?
2. How long has is the pain present
3. Any burning sensation in chest?
4. Any nausea ?
5. Is he passing stools normally? Is there any constipation

There are many causes of post operative pain, so it becomes very important to know the cause for pain so that appropriate treatment is started.
I do understand he requires immediate relief, I am sorry for the questions. But they are very much required.

In response to your questions, I will try to answer as completely as possible.
1. Where is the exact location of pain? Belly (intestinal areas), and in the shoulder (left) up into the neck.
2. How long has is the pain present (intermittent every 3-4 hours since yesterday afternoon)
3. Any burning sensation in chest? No pain in chest
4. Any nausea ? yes there has been some nausea as he has been introducing new tastes to his new stomach so some of those do not agree.
5. Is he passing stools normally? Is there any constipation. Yes there is constipation. He has not passed a stool in 3 days.

Greetings

The cause of pain is likely to be due to colonic distension due to impacted stools and also due to residual gas present in peritoneal cavity . Gaseous distension may irriate the diaphragm causing referred pain to shoulder.

I would suggest you the following which will definitely help

1. Bisacodyl( Dulcolax suppository): One suppository inserted into the rectum will cause evacuation of stools and make him feel better. The distension will be reduced and also relieving of the pain
2. Simethicone containing antacid will be helpful
3. Combination of Pantoprozole + Domperidone taken twice a day will relieve him of nausea.
4. Antispasmodic: Tablet Drotaverine will be helpful in relieving intestinal spasm.
5. Soft diet
6. Avoid caffeinated beverages/ carbonated beverages
