What Causes Persitent Fever And UTI?

Question: I was detected with persitent fever and UTI
I am a diabetic with 8 years history
On investigation Urine culture showed no growth
Prostrate was found mildly enlarged for age 40 years (volume approximately 21 cc)
I visited a foreign country Srilanka 15 days prior to falling ill. Where I went to swimming pool 2 times
I was given Meropenem for treatment. My wife says this is Sexually transmitted disease as fever for persistend and doctor asked this question that whether I had sex outside marriage which I denied and which never happened.

Please advise

Thanks for your query,based on the facts that you have posted it appears that you have persistent fever and UTI.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have difficulty in passing urine (Dysuria) ,Burning urination ,frequency for urination etc?
2) Do you have pain in perineal region?
3) Is there any discharge from urethra?

Persistent fever with symptoms of UTI could be due to Acute Prostatitis .

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Ultrasound scanning of abdomen and pelvis.
2) Swab test and culture of urethral discharge collected after digital rectal examination
(Prostatic massage).

In the mean while take broad spectrum antibiotics like Cefexime with anti inflammatory drug like Diclofenac twice daily.

Ensure to drink more water.To keep your urine dilute This will help to control dysuria.

Ensure to avoid sexual intercourse till it cured completely.

Pain in Passing Urine - No

Burning Urination - Hot Urine (probably due to fever) but no other difficulty
Frequency of Urination - High

From Ultra Sound of Whole Abdomen
Prostrate is mildly enlarged for age (volume approximately 21 cc) and shows homogeneous echopattern
The Urinary Bladder is partially distended with echo free contents. No evidence of any vesical calculus or mass is seen.

Urine Examination
Quantity 10 ml, Colour - Yellow, Appearance - Clear, Reaction Acidic, Albumin - Trace, Sugar +
Microscopic Examination Pus Cells 8-10, RBC Cells Nil, EP Cells 2-3, Casts - Nil, Crystals Calcium Oxalate (++)

How can one distinguish a non sexually transmitted UTI with sexually transmitted UTI . Because this differentiation is important for my case and only this can solve questions on my married life

Thanks for feedback.
In UTI due to Sexually transmitted disease there is always a purulent discharge from urethra associated with severe burning urination which ion microscopic examination reveals presence of Gonococcal Bacteria on routine urine examination and swab test.

Where as in simple UTI there are pus cells detected on routine urine test.
Hope this will satisfy your wife and resolve the issue .

You can get this confirmed by doing screening test for STD (Blood Test)for her satisfaction and to remove doubts in her mind.
