Brief Answer:
please see details.

Detailed Answer:
Dear sir
Pain coming only after lying down and not present during exertion, for one year, can not be anginal pain. Postural variation of pain also speaks against anginal pain. Gastric issues and muscular pain are important possibilities. Normal ECHO tests provide reassurance about absence of structural heart disease.
Beside visiting a gastroenterologist, you must eat at least 2-3 hours before the bedtime. Do not overeat at night. Avoid fatty meals at night. Avoid drinking water with meal. You can take water prior to meals.
A local muscular pain is well appreciated on touch. You can pinpoint with finger the maximum point of pain. If there is any such point or region a physician or orthopaedics opinion should be sought.
Feel free to discuss further.
Sincerely
Sukhvinder

Thank you for your reply Doctor ,

Should i do more Cardiac Related tests , within one year or 6 months of ECHO which was normal then , can i have problem now in heart with 6 months to 1 year time, this is just anxiety , when it pains all i think is only abt heart and get worried , because i get pain in left hand too .
Or Lifestyle change is the only possible solution to this.
Please can you suggest some gastric tablet or muscular pain tablet .
I am overweight , around 88 kgs , 5"7 is my height - FYKI ..

Brief Answer:
consult a physician beofre starting tablets.

Detailed Answer:
Dear XXXXXXX
1. Historically, possibility of cardiac pain is not there, however, to rule out cardiac cause get a prescription of an imaging based stress test from your cardiologist e.g. Stress ECHO or Stress MPI scan.
2. There is no urgent need of starting muscle pain or gastric tablets. It would be more prudent to consult a physician before starting any tablet. Meanwhile carry out the life style modifications I advised you.
3. Definitely you are overweight and must try to reduce weight.
Feel free to discuss further.
Sincerely
Sukhvinder
