What Causes Persistent Body Odour?
I have blood work done annually to check liver and kidney function and the test shows everything is normal.
When I get these comments about my body odor is usually when I get a hot and tingly feeling. I have tried diet changes, avoiding alcohol, caffeine, onions, etc. Still not sure how a sinus issue can cause this, so here I am. I need to make an appointment but not sure if they will take me seriously. I have also seen a dermatologist, who thought this was a funny situation.

It's Dr. Hanif with a warm welcome. Thanks for visiting and trusting HCM. I have thoroughly reviewed health query you raised, and well realize all of those concerns.
PROBABLE CAUSES:
If only you feel the smell of your sweating, then it is good to have ENT examination and investigations done. As the online doctor told you to go for an ENT examination, it was the reason in his mind that if you and only you smell that odor, then there can be an issue with smell receptors in nose area that carry smells to brain higher centres. Usually sinuses around the nose and nasal cavity itself contains some pathology that causes smell perception issues. It can be chronic infection of the sinuses, nasal mucosal dryness, nasal septal deformity (DNS), nasal polyps etc.
If you and other people around you, all feel this odor then it can be due to some metabolic disorder. Then you need to go for an ENDOCRINOLOGIST consultation who is the personnel trained to handle such issues with success. The reason behind this issue can then either simply be higher metabolic rate of body OR it may be due to thyroid disorder.
In higher basal metabolic rate persons there is excess heat production that causes sweating and that is the cause of smell from body that sometimes bothers the patient himself too in addition to others around him. This is usually genetic and less is needed to cure it. We can mask the smell by using deodorants, roll on antiperspirants, having a bath daily, shaving armpits and undershave on regular basis not allowing hair to be visible even. Hair follicles have sebaceous glands that secrete Sebum causing smell in hairy areas.
If this is due to thyroid as happens in Hyperthyroidism your Endocrinologist will tackle that too with proper care. You have the "tingly feelings and getting hot", and these point towards Thyroid issues. But needs investigations.
Diabetes also causes smelly sweat. You Endocrinologist also will have to screen you for diabetes mellitus.
Anxiety can be ruled out by your primary care physician. You have to discuss this aspect of the issue in detail with him on next scheduled appointment. To me all of this isn't funny at all. You have genuine health concern that needs proper investigations.
MANAGEMENT OPTIONS:
It goes then according to cause after consultation from Endocrinologist and/OR ENT specialist.
Keep a balanced and healthy diet. Onion is good for bad sweat odor. It is bad though for causing bad mouth breaths (Halitosis). You need to take more vegetables, salads and fruits. Meats produce more trouble in such cases. Seafood is good for you to take as proteins.

Welcome back to HCM with a follow-up query. I feel sorry for all the inconvenience you have to face with your Primary care physicians (PCP). It is definitely not a good experience.
What you have to do is when you go to your PCP, fill out the sign in at front desk, write down your purpose of visit as "Need a script for referral to an Endocrinologist".
When the nurse gets in the room, just ask her to update the purpose of visit the same as you gave at front desk.
Do the same with your Physician when he finishes with the routine examination and workup. He will definitely go for a referral as there should be no compulsion on part of primary care physician to refer a patient to a specialist. When you will discuss the probabilities aforementioned regarding your health problems, your PCP will listen to you for sure. If he will feel there is no need for the referral, ask him to rule out the probabilities as mentioned in my previous answer.
If that doesn't work, call the helpline of your health insurance company and ask them for referring you to the best Endocrinologist that should be on their pannel.
If nothing seems to be working, just call the front desk of Endocrinologist near your place after searching online or just taking phone number from your Insurance company. Make an appointment and ask the representative to take a referral Script from your Primary care physician office. If they ask, give them your PCP detailed contact information.
