Painful urination with itchy skin and dark urine in a 50 years aged male strongly suggests Genital Candidiasis associated with UTI.
I shall be happy if you can post me more details to help me in making precise comments
1) Are you diabetic?if yes is it under control?
2) Are you able to retract your foreskin over glans penis freely ?
Get following basic tests done to confirm the diagnosis .
1)Urine routine and Urine Culture
2) Swab test of a discharge from penis ?
3) Blood sugar tests on empty stomach and 2 hours after lunch
Start taking
1) Cefixime , Nitrofurantoin and Fluconazole twice daily later on switch on to appropriate antibiotic as per culture report .
2) If turns out to be Diabetic ensure to control it with oral anti diabetic agents as per advice of your Physician or Diabetologist
3) Ensure to wash your genitals with warm water twice daily
4) Ensure to drink more water to keep your dilute .
