What Causes Pain In Right Testicle?

Question: Hello. I"m a 38 year old male non-smoker with no history of cancer in my family (beyond lung cancer in my elderly grandmother, but she was a chain smoker).

Anyway, I went to the ER for pain in my right testicle. It wasn't an excruciating pain, more of an uncomfortable squeezing sensation. The sonogrammed my testicles and found nothing wrong. They did blood work and a urine test and the blood work came back perfect and the urine test was fine except for a microscopic amount of blood, of course unable to be seen with the naked eye. They released me and told me to follow up with my doctor in 3 weeks.

In the next 3 weeks, the pain in my testicle went away, but then returned. I went to the doctor and they drew blood and urine again and the results were the same. All was perfect in the blood and urine except for the same microscopic blood in the urine.

I'm waiting to see a urologist but I dont know when that's going to be due to insurance company red tape. In the meantime, I'm asking for some suggestions on what I could be dealing with.

I'm still seeing no blood in my urine with my eyes. I have no pain or burning when I urinate nor am I urinating more frequently or stopping and starting and I'm not feeling like my bladder isn't emptying. The pain in my testicles does come and go from time to time but it isn't as intense as it was in the beginning.

Can someone please give me some insight?

Hi.

Thanks for your query and an elucidate history.

(The pain in your right testicle started 3 weeks back. It wasn't an excruciating pain, more of an uncomfortable squeezing sensation.
The pain in testicle does come and go from time to time but it isn't as intense as it was in the beginning. tests normal except traces of blood in urine. No urinary complaints )

As the tests are normal except the microscopic blood , one thing of for sure there may not be a retrograde infection from urinary system to the testes.

Such type of pain can be due to:

Mild form of orchitis, that is inflammation of the testis and epididymis. The nature of pain is dull aching. The pain can elicited on mild pressure../ Remember all the testis has a peculiar sensation on pressure. IN this condition there can be a pain even on touching the testis.

Can be due to varicose veins, meaning dilated veins evolving around and above the testis. This also causes pain, the nature is more of a dragging type. One can see the bulge like a bag of worms only in standing position and when you increase intra-abdominal pressure by straining. You can elicit this on your own

If the nerve supplying the testis is stretched by hernia or entrapped in its course
The nature is more of a changed sensation and/ or burning

I take this in a way as you have not mentioned : there is No fever, No severe pain anytime, Not causing any disturbance for your your daily activities. There is NO local swelling.

Please co-relate with the suggestions I have given above and you can check a few on your own.

The best way is to give a '' Scrotal support'' by Lungot or underwear like a brief where you can SUPPORT the testis at a higher level ( not to press against the body).

I would advise a repeat ultrasonography with a color doppler examination. You can discuss about the tests with you visit the urologist.

Yes. There is no fever. No swelling and even when the pain is there I can go about my day without issue because it is more a squeezing sensation than a pain. My first doctor suggested that this recurring pain combined with the microscopic blood in the urine could be small kidney stones.

Does that make sense?

Would that account for blood in the urine?

What about an enlarged prostate?

I hope Ultrasonography of the abdomen has been done. Get it done if not yet done. This will help to find or rule out the stones in urinary system, will be able to detect any abnormality in the whole system including the kidneys, ureters, urinary bladder , prostate and seminal vesicles. It can detect even the small stones. It may help to find the cause of microscopic blood in the urine.

Testicles can have referred pain only if the stone is present in the 'lower ureter' and not in any other part of the system. The pain in such cases is sharp troublesome.

If there is crystalluria- a condition wherein there is formation and passage of crystals, the sonography findings of abdomen may be negative and there will be definite symptom of burning micturition and finding crystals in the urine examination ( which is negative in both the reports).

So the ''squeezing pain'' can be due to local problem as explained and needs a review ultrasonography of the scrotum with color doppler examination preferable during the the episode of pain. Another possibility of such type of pain is partial torsion of the testis. During the upward elevation of the testis ( occurs during every bodily act which involves the abdominal musculature) there is partial torsion. Your Doctor can detect this during examination in standing position and asking you to cough, simultaneously palpating the testis.

Blood in urine without pain in abdomen can be due to multiple reason. May need added IVP ( intravenous pyelography) / CT and so on- to rule out the possibility of a mass or so. It is not possible that every case of microscopic hematuria that one can find the reason.

Prostate pain is different that that you have. And can be found out on digital palpation by a Doctor and/ or Ultrasonography.
