Question: Within the last 2 weeks I have been hit with a lot of pain. I'll give you a brief summary: New lower back, numbness in hands and feet, lots of cracking and popping in my joints, Ongoing neck pain, left shoulder pain, right ishium tuberosity pain, headaches, elbow and forearm pain... The numbness to this degree is new, hands and feet are always cold and bluish purple at times,. I have had epidurals in my neck for neck pain in the past, about 1 1/2 months ago had facet joint injections in my neck, the elbow pain has been going on since last autumn, ishium tub. pain has been going on for over a year - on and off (no dr can figure out why). I have also had issues with arthritis and am on arthritic 50mg and plaquenil...I just increased my dose of arthritic to 100. They are treating me for a non rheumatoid factor RA but I also have symptoms of osteo. My fingers are usually always at least a little swollen and I do get a small amount of edema in ankles and feet. I'm just tired and frustrated because no one seems to be able to "fix" me. I don't know whether to see a rheum or an orthopedic dr. Im sorry if this seems scattered but I am just so desperate to feel better. I am 5ft 4in and weigh 120lbs.

Brief Answer: Opinion of a rheumatologist Detailed Answer: Hi and thanks for the query, the symptoms you describe are very compatible with a rheumatological condition. Rheumatoid arthritis is very compatible with your clinical symptoms. Numerous drugs do exist for treatment of rheumatoid arthritis but required planning and step wise approach. Plaquenil is a treated of choice especially when the condition is not responsive to most first line drugs like non steroidal anti inflammatory drugs, steroids, methotrexate and gold salts. It side side effects and your rheumatologist usually does a proper review before putting you on this drug. I strongly suggest you book an appointment with a rheumatologist. Kind regards as i wish you the best of health. Please, do feel free asking further questions in case of need. Kind regards. Dr Bain
