What Causes Pain In Joints Of Knee And Legs?

Question: from last couple of weeks i started feeling pain in my joints started from my knee and below, my mother has feet joint pain severe, which i believe is arthritis but not sure what type of that arthritis is. Anyways i went to GP last week and he asked for some blood test, which i attached here as 'full blood test', which seems to be inclusive and currently i am on augmentin for my ear infection, panadol osteo (which is panadal 635 mg), but my pain is not coming down and almost spread to all of my joints and it is too severe during night that i cannot change my side while sleeping. current blood reports were examined by local GP and he said inflammation is there and some sort of viral or bacterial infection is there, but not able to conclude what actually is the issue. he mentioned to another XXXXXXX GP before referring to me to a rheumatologist. so i need help to understand these blood report and what need to be done. please take a note that i am suffering from sinus and middle ear infection with right ear blocked with liquid trapped behind the ear. Also blood test showed that i have dust mite alergy tool

Thanks Dr XXXXXXX for your response, currently i am confused with below statement as it talks about more than one issue, may be after providing specific answers to your queries will nail it down to 1 issue:-), so pls find my answers below

A1. Yes, it last for many many hours and currently i am taking panadol Osteo (paracetamol 635 mg thrice a day), and it helps a little.

Q2. Bilateral symmetrical involvement of hands and feet joints
A2. I am not sure what exactly it means, but i am assuming whether pain happens on both feet joints, or both shoulders instead of 1, then answer to this question is yes.

Q3. Hand joints and feet joints that are warm and tender to touch
A3. yes
Q4. History of viral illness like chikungunya.
A4. No, i am in melbourne from last 5 years, so no mosquitos over here.
Q5. Flu-like feeling
A5. Yes i have flu like feeling, but as i mentioned i am also suffering currently from sinus and middle ear infection also. and that may be because of that too. anyways i have pain in my jaws, teeths as well, and i am currently sweating at night, may be because using vicks vaporub to keep my nose (sinus effected) open and contribute to sweating as well, also when i wake up at night for bathroom, i can also feel chills too, and by the time evening starts i can feel feverish too.

A8. currently yes after suffering from this problem, but otherwise in general no.

Dr XXXXXXX may i request to pls respond to my queries as soon as you get some time, also while talking to an ENT on this portal, he advised as below, therefore could you suggest whether this is the right approach and any other suggestion you have besides responding to my previous response to you:-)

"The ANA report is not conclusive. But ESR and CRP both are raised .
What i suggest is get an XXXXXXX (anti–neutrophil cytoplasmic antibodies) test done to rule out Wegeners Granulomatosis. In addition get done Urine examination for casts and RBC ; Chest X ray posteroanterior view.To rule out Wegeners granulomatosis."

Please take a note that yesterday night was also too bad for me and i was not able to sleep well due to pain in various joints and was not able to change the sides easily, and during the night i took panadol osteo (paracetamol 635mg) tablet and then i felt a little fine.

do you what other type of medicine can help me so at least i can go to office and join my work, currently i am at home from last 2 weeks due to the joint problems and still doctors has to find the issue and then only put me on right medication. pls note that here in australia no medicine is provided without the prescription of the doctor, so anything you will mention, i need to talk to GP and if he feels that it is the right medicine only then he will give that to me.

Sorry for late reply,

Your symptoms are very much suggestive of rheumatoid arthritis. After confirming the diagnosis, with a few tests(like Anti CCP antibody test ), you have to be started on medication called disease modifying anti rheumatoid drugs(DMARD) alone or in combination with biologicals (Immunomodulators). Since these are prescription drugs and regular follow up would be required, it would be best if you see your general physician for the same.

You can continue Vit.D and calcium supplements. Pain killers like Indomethacin 25mg can be taken to reduce stiffness and pain. Routine physical activity is always advised as this will prevent stiffness from aggravating.

I would also like to give some advice to make the life better and to fight with these pains-

Do regular physiotherapy of joints.
Hot fomentation to decrease the joint stiffness
Hot bath of Epsom salts (magnesium sulphate)-Magnesium has both anti-inflammatory and anti- arthritic properties and it can be absorbed through the skin.

Thanks once again just a few follow up questions
1. From yesterday night got pain in my right hand fingers and i cannot open door knob or hold any metal thing tight as i feel intense pain, is this indicates the same problem you mentioned?
2. Also i never had any pain issues and i just got this aggravated sudden issue from last 2 weeks then how it is possible to have this big issue? That is a XXXXXXX ?
3. Is this just not be a viral or bacterial infection and also i have pain in my jaws and teeth, is this because of the arthritis
Additionally could you pls how much vitamin d and calcium to be taken daily?
further I visited the GP today and he suggested for repeat CRP, ESR & FBE with fasting this time and with some additional test also ANF, DNA binding and some others (sorry cannot read doctor's handwriting)

Hi,

It could be a just viral or bacterial infection, but your symptoms are much more suggestive of rheumatoid arthritis. Rheumatoid arthritis can present like this even in a short period duration.
I suggest you to get it examined by a rheumatologist for proper diagnosis and management.
1,000 milligrams of calcium per day and 400 i.u. of vitamin D per day.
