Brief Answer:
need further details for assessment

Detailed Answer:
Dear
Thanks for contacting Ayurveda with your health concern and after going through your medical history some further queries need to be answered for ayurveda management.

1. Duration: since the disease is rare, benign ( non cancerous) and chronic thus it is favourable in your case, as such do let me know about the time period?

2. Since CT is suggestive of 'mesentric panniculitis' which means at present there is presence of inflammation and fat and not fibrosis. (again a good sign)

3. Since aetiology is varied and mostly unknown but in 80% cases h/o surgery or trauma is present?

4. Since case is diagnosed, and investigation done, was any opinion of Gastrointestinal doctor sought?

5. Do let me know about your current/past medication reviewed/received to prevent any drug interactions.

6. Latest study have shown strong relationship between addiction and panniculitis. .any such?

7. Do revert back for further necessary assistance.

Brief Answer:
one has to consider different aspects

Detailed Answer:
Dear
Thanks for reverting back.

1. Since you are purely vegetarian, addiction as cause can be safely excluded and since time period is 20 years, hopefully the disease cannot cause any sudden change in physical examination and system.
2. Since steroids is/are the empirical treatment in allopathy, you are the best judge. [since there is no standardized treatment]
PS. Resection/surgery is advised if ongoing inflammatory changes become 'irreversible' and/or there is bowel obstruction.
3. Since I'm not specialized in Homeopathy thus kindly consult one in your area for further assistance for Medicine and management, But being an Ayurveda Physician I usually manage the patient on the line of: 'Amaj Shoola' since there is no direct reference of the disease in any of the texts.
PS. Since shoola [pain] can be a symptom or complication of abdominal disorder which perfectly fits into your pathology.
4. In Ayurveda we believe that healthy 'stomach' [amashaya] of person cures abdominal disorders, thus I usually provide medicines after giving Panchkarma procedure to the patient and thereafter the line of treatment is:
. Kshar prayog [use of alkalies]
. Deepan-Pachan [appetizers and digestant]
. use of Virechak [laxative] and vata anuloman [carminative] properties.
PS. medicines are: kshar gutika, jambeer asav, shankh vati XXXXXXX kshar pachan churan, panchskar churan.
5. Since no two person have similar 'Prakriti' [constitution], thus cause and effect is different from person to person, and thus treatment and prognosis vary

Brief Answer:
Gomutra Therapy.

Detailed Answer:
Dear
Thanks for reverting back.

1. Since it is told by great scholar of Ayurveda, Acharya charak that cows urine [go mutra] can be taken in abdominal problems [gulma], studies and international journals have validated its true concept.

2. to be on the safer side, I usually instruct/tell my patients to drink distilled cows urine and not raw. [since cow can be infected with some disease most of the times], and this form is easily available in market by name of 'Go Ark'/ kamdhenu ark or you can prepare it by yourself at home, but keep two things in mind:

i. Cow should not have given birth to calf
ii. urine should be collected before sunrise.

3. Initially you should take 4-5 drops of cows urine twice daily and after a week the dose can be increased.
PS. keep in mind that;
. If urine collected is fresh it should be consumed within 10-15 minutes
. If urine is distilled [preserved] it has shelf life period up to 4-5 months
. You can ask for XXXXXXX gutika, gomutra haritki, mandoor vatak where cow urine is an ingredient or you can take 'kankayan vati' which is to be taken with cow urine as Anupan [co-drink]

4. If we go into the nephrology: cow's urine is basically filtered cow's blood (and filtration is done by the kidneys) so is an ideal choice for people with chronic kidney diseases, one negative effect which is seen/complained by most of the patients is/are: gomutra induced rashes and itching [if this happens you might have to discontinue]

Thanks for reverting back.

1. Since you are in stage of fat necrosis, no treatment is recommended, except immune suppression therapy and later surgery.

2.
i. Have single dose of Ornithogalum umbellatum [mother tincture] and wait for action
ii. Take Colchicum Autumnale [in a dose of third to thirteen attenuation] but under supervision

Brief Answer:
Clarification and Suggestion

Detailed Answer:
Dear
Thanks for reverting back.

1. Since condition is diagnosed as mesentric panniculitis which is an 'inflammatory' condition, which can be the cause of the pain, at the same time since you are suffering from last 15-20 years, thus formation of calcified granuloma although does not cause pain but can be an aggravating cause.
PS. but this can vary depending on the person and the area where it is located, so your attending physician can help in such situation.

2. I have always advised my patients to take fresh go mutra with honey and lime juice in morning since it enhances metabolic activity of metabolic activity of cells and tissue.
PS. never have/had experience with turmeric/punarnava, and there won't be any side effects [other than mentioned earlier]

3. Diet:
Do's: dry ginger, soup, radish, lemon, old brown rice, protein rich diet, butter milk, horse gram, harad, bitter gourd, adequate rest
Dont's: salt, citrus, meat of water/land animals, newly harvested rice, jaggery, curd, sesame seeds,alcohol,heavy/spicy/slimy food, oil, alkalis, day time sleeping, excess sex indulgence.

sir
I want to know

1) To cure the mesentric panniculities(MP), first we must take homeopathy or gomutra, and is homeopathy having any side effects and disadvantages to the body.
2) How many times and how much quantity we must take gomutra with honey and lime.
3)we are having a facility to have black cow urine daily (which has not given birth to calf ) so that we can take the raw cow urine mixed with hot water, if so how much quantity and how many times shall i take to cure the disease.

4. I welcome your opinion and suggestions.

Brief Answer:
Gomutra and Clarifications

Detailed Answer:
Dear
Thanks for reverting back.

1.
i. As urine is ultrafiltration and homeopathy is associated with dilution
ii. As urine is given in drops so does homeopathy depends upon potency.

So gomutra itself can be regarded as a Homeopathic medicine with negligible side effects.
PS. there might be initial aggravation symptoms followed by relief afterwards i.e. if you have loose stools 3-4 times per day, it will be physiological action of gomutra [cleansing of intestine]. or you may reduce the dose to half it it continues a few days more, later on you can again switch to full dose.

2. In one glass of water:
5 to 10 ml [50-100 drops] of gomutra
1 tsf of lime
1 tsf of honey.. morning [7:00 a.m] and evening [5:00 p.m]

3. urine should be taken 10ml with water; early morning and at bed time [night]
