Posted on Wed, 26 Aug 2015
Question: my question is this I have had severe electrical shocks in hands and feet plus weird nerve pain where my fingers go numb the pain last about 10 seconds I also get this pain in my feet and toes and sometimes in my eye. My neuro on a consult with gp suggest PA looked up symtoms. Sure enough I have almost al of them including white skin spots on forearm to white spots like sore in mouth. A serum b12 was done with 720. I ask them to do test again cascade with MMA and homocysteine. They refused. I am sure I am deficient. THIS HAS BEEN GOING ON FOR 5 YEARS. I am sure I have nerve damage. I do also have severe spinal stenosis but believe that has no bearing. Also on Coumidin. I was promised a B12 shot then they backed out last week, I also having depression issue which I am sure are from low B12. I also asked for a HoloTC active B12 test they refuse to take me seriously. They love giving me pain killers which I refuse to take unless pain unbearable that level for me is 8. I take Hydrocodone and Oxycodone. I was put on Trazadone for severe insomnia. Only been on it 3 days. Saw my psychiatrist today cause I score bad on 9 question psych test. No Shit I sick of pain and nobody listening to me. The most Scary part is this is Mayo Clinic in XXXXXXX MN, Soon to be Destination Medical Center. One other thing is I am XXY/Intersexed and have been on estrogen for 4 years. I guess you could say I am a Male to Female trans person who is very adjusted and have no issue with that side of me.

Brief Answer:
EMG needed to be done.

Detailed Answer:
Hello and thanks for your query here at HCM.

I have read your query and understand your concerns.

Symptoms you describe seems to be related more to peripheral neuropathy and low vitamin B12 is one of several causes that may cause it.

EMG done 6 years ago isn't relevant in the present, so, in my opinion, a new EMG is necessary at this moment.

If you have psychological issues you should address to them by consulting a Psychologist, but neuropathy related symptoms should be solved first.

If strong evidence of neuropathy after EMG study, treatment about it should be started as well.

Peripheral neuropathy symptoms respond in a satisfactory degree to drugs such Pregabalin (Lyrica), Amytriptiline, Gabapentin (Neurontin).

I suggest you to discuss with your treating Doctor about above mentioned possibilities.

Hope you found the answer helpful.

Take care.

I was on Gabapentin about 4 years ago. If fact I had to work my dose up to the highest allowed something like 2700 mg. I started on lower does for the nerve pain but slowly became immune to it, or it was not effective so they kept increasing dose. Of course it took some time for me to adjust to the sleepy side affect. Then after a year a lot of nerve pain went away went down to a pain level of 2 or 3 and that kind of my normal almost like (0) for me. I saw my psychologist Tuesday he wants me to see endrocrinologist thinks I may have a estrogen issue since I am intersexed, trangendered. I'm sure this is not the issue I have had these pains long before I was on HRT of which is 1mg estradiol 6 days a week and one day 2mg sublingually. I did find methylcobolomin liquid at our local good food store and start that yesterday over taking cyancobolmin. Please correct me but if I have had long term nerve damage that may or may not fix itself and can take many months or years to repair. We also decide to split my clonazepam from 1 .5 mg per day to .25 3 times a day. a half pill at morning, dinner, and bedtime. The trazodone has helped a lot with the insomnia other than a low grade headache in morning (not a migraine) Tylenol 2 500mg make it go away. plus it a depression med as well which certainly will help me. I swear the B12 supplement is doing something the severe pains in hands is changing I still have pain at a reduced pain scale like 4 instead of 8 and it feels different. I refuse to take oxycodone unless pain is over 8. Psychologist thinks there could be some other depression issues although I do not thinks so I am to report back to him 7 days via Mayo's patient portal. As much as I don't want another EMG I will take your suggestion to GP which already wanted to do this and see if I can get that scheduled next week. Psychologist also suggested I might want to start taking 300mg Gabapentin times a day. Got plenty of that around here. My fear is this I don't like masking symptoms till I have a known cause or complete diagnose. Thanks for your help I sure I will be asking again for some input. XXXX

Brief Answer:
EMG is the key point here.

Detailed Answer:
Welcome back XXXXXXX

It is good to hear that you agree to have a correct diagnosis, and EMG is the key point here.

It will quantify the nerve damage degre and will give a clue of the recovery period.

The fact that cobalamin is helping is reassuring that improvement is possible, but it will take some time.

Glad I helped you.

Take care.
