What Causes Overnight Thick Urination?

Question: I AM SUFFERING FROM BIOCHEMICAL RECURRENCE AND A HERNIATED DISC. I PEE IN A RECEPTACLE AT NIGHTS AND RECENTLY IN THE MORNING I NOTICE A THICK JELLY LIKE DEPOSIT IN THE RECEPTACLE. IT IS THICK AND ROPEY AND DOES NOT MIX WELL WITH WATER. CAN YOU SUGGEST WHAT THIS MIGHT BE

Re the biochemical recurrence,I had RP in 2007. Please see PSA test results.Is there a possibility that the cancer is attacking my bladder internally, resulting in the thick ropey stuff?

I had Radical Prostatectomy (RP) in November 2007
5 months after RP, my PSA was 0.07
11 months after RP, it was 0.15- (11 months to double from 0.07)
28 months after RP, it was 0.36- (17 months to double from 0.15)
38 months after RP, it was 0.45
56 months after RP, it was 0.62
64 months after RP, it was 1.20 - (12 months to double from 0.62)
76 months after RP, it was 3.02 - (14 months to 2.5 times from 1.20)
80 months after RP, my PSA has skyrocketed to 8.61- (4 months to 2.8 times from 3.02)

Of interest to note is that for the last 24 months, I have been plagued with recurring UTI on at least 10 occasions
This coincided with the beginning of the rapid rise in PSA level after 56 months.
My urine has been permanently cloudy, frequent urination, small amount and sometimes painful.
My last PSA test and Urinalysis were done XXXXXXX 9th, instant: PSA (Total)-8.61, PH-6.5, SG-1.025,
Protein –Trace, WBC- 5, RBC- 0, CULTURE – GPC <10,000

I am so sorry to hear about this recent observation in your overnight urine samples and the rising PSA. We would try to take a closer and logical look at these to understand what the exact problem is and propose meaningful explanations or solutions.

1. For the overnight "ropey" and thick urine, I will like you to drink much fluids, stay hydrated and monitor again. This is very non specific and not suggestive of any specific urinary tract disease. However, a high concentrated urine may predispose to sedimentation and formation of thick deposits that you have described. By drinking enough water, we would be able to gather some more information about this and make a decision on what the next steps are.

2. Your rising PSA levels are of concern but ,am not sure this is related to cancer of the prostate. What was the stage of your prostate cancer at the time of surgery? Was it confined to the prostate or was there evidence that it had spread to other organs? Also, was your surgeon able to completely excise the cancer cells? PSA, has many other factors that can cause an increase in its levels and not necessarily by cancer of the prostate. One of the common causes is urinary tract infections. The fact that these coincided makes me think it is more likely to be related to urinary tract infections and not a cancer recurrence.

3. Recurrent UTI signal incomplete or inadequate treatment or the presence of a predisposing factor to have repeated infections. Talk with your doctor to check if the antibiotics and duration are adapted to the isolated germ or whether there are other predisposing factors that need to be addressed.

4. If your doctor cannot provide answers to this, please visit a urologist for a full evaluation of the bladder and urethra. This will look for possible reasons for recurring UTI's and address them. I will not mind should it be that you need a cystoscopy, bladder studies, urethra imaging, etc as long as the exact reason for repeated infections can be identified and managed appropriately.
