Question: adult 71yr male first time thrush diagnosis...is it a definite hiv infection 100% or other diagnosis...it appear s on my tongue

HIV should be ruled out.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney.
Oral thrush / Candidiasis that too extensive warrants exclusion of HIV but it's not that it's 100% because of HIV infection.
Another very common reason for it is Diabetes.
Also people who are on any immunosuppressive drugs like steroids can get such oral thrush.
So, I would suggest you to get HIV and Fasting Blood Sugar tested.
