Question: On exertion my heart sounds are constantly in my ears? Have only ever experienced this if i had a fever

Based on your query, my opinion is as follows:

1. Any exertion is similar to fever. The body during fever will be functioning at a higher metabolic rate with increased oxygenation to improve the overall function/ remove the cause of infection.
2. It increases the blood pressure and heart rate, which can be felt as the blood moves with greater force through the blood vessels.
3. Check your blood pressure once. If normal, not to worry. Avoid excessive exertion. Along with it, if you have breathlessness or chest pain, you will need to meet your doctor to get a Stress test/ treadmill test (stress ECG) done.
