Posted on Wed, 5 Apr 2017
Question: How can I gradually get off the pain medication I'm taking which is 'morphine sulfate SR' ? I've been on it for 3 years from having neck surgery. I tried taking 20 mg. a day but I started to have pain and nausea. What can I do??

Brief Answer:
rather a lot of options.

Detailed Answer:
That would be better handled if one had a physician who knew about the situation. There is rapid tapering suboxone, there is tapering the morphine but taking medicines for nausea and shakes (zofran or phenergan for nausea; clonidine for shakes). There are rapid detox centers that sedate you totally (like XXXXXXX Jackson) while you come off of drugs (sometimes with the same result as with XXXXXXX Jackson).
One pill a day is not enough generally to produce withdrawal.

But taking just one pill a day brought on withdrawal, so I should I ask for the Suboxone then?

Brief Answer:
You didn't say the amounts.

Detailed Answer:
While I do not give specific advise unless I am directly there, I cannot give general advise without knowing about how much narcotics we are talking about.
"y. I tried taking 20 mg. a day but I started to have pain and nausea. What can I do??"
The general possibilities (that's about all of them) still applies. But would need to know the ballpark amount we are dealing with.
2 x 20 mg a day is not that much and generally would give mild withdrawal lasting up to a week. Generally not even that long, but it is a timed release form. Still, cutting down by about 20% each week with medication for the symptoms would be the method most commonly used for slightly high doses of narcotics.
If someone were on 5 to 10 20 mg pills a day (100 to 200 mg) then suboxone would generally be used.

I take a 15mg.in the morning and the same at night so 30 mg. total. Could i still ask for the suboxone then?

Brief Answer:
tricky

Detailed Answer:
In general 30 mg a day is a low average dose without much problems with dependence and withdrawal. People vary and more drug effects from a given amount would tend to occur in older, smaller, and female people.

The standard suboxone dose is nearly 10 fold more drug equivalent.
They MIGHT do the correct dosing the way the text book says and start on SLIGHTLY more per day 1.5 to 2x. but then you'd be at the dose where standard protocols would say to just stop.

So, if the suboxone clinic is really motivated to help people and creative they'd adjust dosing according to what amount of drug people were previously on. start you on 2 mg suboxone (it is 20x more potent than morphine) which would be about 40 and you could cut it in half and take half twice a day (total 2 mg 40 mg equivalent per day).

This is a small quantity and further lowering of it gets tricky because the films are getting cut exceedingly thin... But it can be done and we've had success many times.

So, subxone is one option
alteratively giving medicines to lower the symptoms and just drastically lower or stop the medicine.
Note: the FDA guidelines START at 30 mg per day and below that is not generally considered all that much to require attention.
