Question: About once a month I have a day where I am really nauseous and am told I smell metallic. What could it be

I have gone through your query with diligence and would like you to know that I am here to help. I understand how this presentation can be slightly worrying and sometimes even frustrating, which is why I shall list all the possible conditions that can cause a metallic smell (mainly from the mouth) and which one of them is most likely when you have another symptom(nausea) along with this bad breath. Let me start by listing the possible causes:

-Gum infections (gingivitis)
-Dehydration
-Medications
-Conditions like dry mouth syndrome, sarcoidosis, Sjogren's syndrome, etc.
-Poisoning (eg. iron poisoning, etc.)
-Kidney abnormalities
-Stomach infections

In my opinion if your metallic smell is indeed from the mouth and nausea is present at the sometime the most likely cause is dehydration. I would recommend increased fluid intake and regular small portioned meals (eg. bowl of vegetables, one fruits, etc.). If at all you do not have any other symptoms associated with this presentation, and if these symptoms last only for a day (24 hours) maximum and you seem to fully recover within 24 hours, then dehydration is the most likely cause and nothing else sir.
