Question: Hi. I have had a low grade fever between 99.5 and 100.7 for a couple of months now and increasing back pain the last month. I have had a small amount of blood in my urine that the lab detected and WBC count slightly elevated. I have some nausea but no vomiting and last urine test didn't reveal bacteria in my urine. The doctors can't seem to figure out what is wrong. I just had a CT test and now waiting on the results. The pain is mainly in my mid back right now and hurts to sit up straight in the chair.

Thanks for your query,based on the facts that you have posted the back ache with nausea,mild fever and presence of blood in urine with raised WBC count could be due to Stone in kidney with secondary UTI.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have any urinary symptoms like burning etc ?

Please get your routine urine test and urine culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.

Please take broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water.To keep your urine dilute This will help to keep urine dilute

Wait for results of C.T Scan .

Further treatment will depend upon result of C.T Scan , urine culture . and final diagnosis.

Thank you Dr. XXXXXXX I have had three urine tests and the first one revealed bacteria, which I was prescribed antibiotics, but I am on my third round of antibiotics now and still having the fever with back pain. The last urine test did not reveal bacteria in my urine but I did have a small amount of blood. The antibiotic (bactrum) doesn't seem to be working to get rid of my fever.

Thank you for all of your recommendations. I will look into getting those medications and will drink more water. I have a tendency to drink too much coffee.

Pain ,fever with blood in urine should be suspected to be due to stone in kidney or ureter which needs to be confirmed .

Go ahead and get the tests suggested done to establish the cause for your symptoms

Thanks for uploading C.T Scan reports which reveals presence of two cysts in liver which are likely to be simple benign in nature.However this can not be a the cause of blood in urine which needs to be investigated further

You have done urine tests thrice but it seems that they were routine urine tests and not a culture. I would suggest you to get Urine Culture done to trace out the cause of blood in urine and take appropriate antibiotic as per culture report .
