Brief Answer:
QUIT

Detailed Answer:
Hi,

You are a smoker, so besides seeing the doctor, the best advise I should give you is to QUIT it. I know this can be the cheapest advice but I bet it is going to the BEST.
I read through the details and found that the blood supply to the legs while at exercise is dropping quickly hence your distance is reducing time on time. There should be clots forming which is common amongst smokers. If it were true on color doppler scan done during your doctor's visit, not any more drugs would help, but quitting the smoke would definitely help.
The initial signs were ED, where the vessels to the penis seem to be blocking and then the smallest vessels to the back are severely affected thus causing pain.
The BP is not looking serious though but definitely improve once you QUIT.

Brief Answer:
Yes

Detailed Answer:
Hi,

Yes color doppler would say that if there is thrombotic clots in the leg veins interrupting the blood supply and thereby restricting the muscles to walk distances.

The specific clinical symptoms that you say are not suggesting pancreatic cancer. MRI of the abdomen can find out pancreatic tumors. Cancer is a pathological diagnosis so only on biopsy would know it.
