Question: I smoked my knee cap into a doorway 2 days ago. Iced it the whole day so I can walk with no problems. Going up stairs it feels sore and I think I can feel fluid. What is the indicator that I need to go to a doctor

Fluid under knee cap itself is the indicator that you need a doctor, because any if cause patellar test to be positive must be aspirated by doctor for culture and sensitivity.

Examining doctor also see the color and consistency of fluid and adivise further treatment in the form of painkillers, antibiotics and cast if he finds blood in fluid.

Blood in fluid indicate that there is some hairline fracture in knee joint, so cast is needed after confirmation of xray.
