Question: I am a 65-year-old woman with an extremely itchy red dry rash on the inside of my right ankle. Help, please!

Thank you for posting on HCM.
Gross from the picture and history I am thinking of Tinea corporis , a fungal infection of skin.
But since you have not mentioned the duration of rash, even possibility of insect bite cant be ruled out.
Kindly revert back with the duration of rash and any other significant history so i can help you in better way.

The rash has been there about two months it is not an insect bite. I thought it might be a fungal infection so I tried Lotrimin on it but it did not work. Started out small and itches so bad I had to scratch it and every time I scratch it, it just itches worse. The itching is very intense especially when I'm scratching it. I tried several different creams and lotions all to no avail. When I'm taking a shower it starts to hurt really badly apparently being wet hurts it. I am putting on lanacain for the pain. The skin gets very dry and cracks that's why sent you two pictures. One of the pictures is when it's dry and the other picture is when I put something on it like a lotion. You can see by one of the pictures that it itches so badly that sometimes I scratch it so that it's raw.

Oral and topical antifungals

Hello again,
Okay now its pretty clear that you are having tinea corporis, a fungal infection of skin.
Inspite of applying various creams/lotions you are not getting response that is because there is no oral antifungal drug in your prescription.
Generally, in my patients I would suggest the following treatment regimen:
1. Oral antifungal drugs like fluconazole or Itraconazole for few weeks.
2. Oral prednisolone in low dose (5-10mg) for 10-15 days
3. For local application, I would suggest a cream containing betamethasone/clobetasol mixed with fusidic acid and a cream containing antifungal drugs like sertaconazole or Luliconazole. Mix both the creams and apply twice a day over affected areas for 10 days. Thereafter, just maintain on antifungal cream for few weeks.
4. Oral antihistaminics to relieve itching and irritation.
5. Maintain hygiene and use ketoconazole soap for daily rinsing.
6 Avoid pricking/scratching/peeling of lesions as it can aggravate the condition.

Discuss this treatment regimen in detail with your treating dermatologist.
I am sure if rightly followed you will be fine soon.
I would insist you to post regarding your condition after 2 weeks.
