Question: Hi doctor
I am 70 yrs old,78kg weight,5.9 height, from XXXXXXX T2D
for the last 12yrs, on tablets, doing good excercises daily, walkin 4 kms daily, follow d diet.My present FBS and PP are 144 and 120, BP 139/70, grade 1 fattly liver.No habits or smoking, drinks.My Sr creatinine fluctuates bet 1 and 1.4.My Sr potassium is 5.2.I am taking Tab Zoryl 2mg 1-0-1/2, Galvas 50 0-1-0,Voglibose 0.2 0-1-0, Telma 40 1-0-0, T Cilacar 10mg 0-0-1, Meganueron OD+ 0-0-1,Sutradyn 1-0-0 daily. I am under a good daibetologist care here and no major health issues so far.Have got retina laser done 5 yrs ago, lefy eye severe itching and floaters there.Showing to Dr N B KUMta here.
MY doc tells me to take care of creatinine, potassium and foot .Though creat, potassium (5.2) are not v v high , i am little worried how to control them within limits.Plus if i eat outside, marriages etc, gets heaviness on right side of stomach.I am a konkani from S Kanara and seeking ur advice on wat aditional care i should take, whether any change in medication needed, any new medicine reqd etc.Forgot to add that i am also taking Fenolip 145 one daily for TGL(now it is 160).Appreciate if u will pl study my case and give ur valueable advice and guidance.

THank you doctor for ur q response.
1 My HBA1C was 6.9 test done in Dec,1994
2 My appetite is very good.In fact I get toxic hunger after meals which I am trying harnd to control.
3No weight loss.I was weighing 79 and because I am doibg more exceracise , walking and d control, it has come down to 77 kg.
4 I have this heaviness since last 35 years or so.Sometime in 1985 or so, barium meal showed duedenum norrowing, so Endoduedenoscophy was done.It showed no narrowing, no cancer, no ulcer etc.Then I consulted well known surgeon DR V N Shrikande in XXXXXXX and he got my gall bladder testdone which turned out to be normal.He said nothing wrong , it is due to .....hypochondria .................and dont go for any further tests orn some doctors may open my stomach to see wat is wrong inside.So no medicines, tests since then but that same heaviness type on rightside near liver area I get, it happens with outside food or excess eating.No pain at all, only mild heaviness and feel like food stucks up and get cleared after 1 hr or so.My bowels are semi solid, sometimes well formed.
Hope I have given full inf available with me .PL look into my case
I have made some spl mistakes in my answers and request you to
read them correctly.My last HBA1C wqs done in Dec,2014(not in 1994) , it was mistyping.Sorry for the mistakes
and help me and advice.

Thanks for the additional information.
Your HbA1C indicates you have good control of your blood glucose during the previous 3 months. HbA1c less than 7 indicates good control of blood glucose.
The medications you have been taking are good enough.
As far as the serum creatinine and potassium are concerned, they are not very high enough to be of any concern.

The heaviness you get in the stomach could be due to increased gas, gastroparesis or functional dyspepsia. If this symptom occurs only occasionally you can just ignore it. But if it is persistent/recurrent/bothersome, you can take medications like levosulpiride for relief.

Thank u for ur quick reply.I want some more clarifications and ask these last questions.I am happy with ur opnion and advice ,especially on gastroparesis.
I dont have continuous heaviness on right side of stomach and it happens only
on certain times if i eat little more or with certain food.I may not require Levosulpiride but still wants to know the details
of this medicine visa-a-visa dosage,strength and any side effects.Can i take it as
SOS in case of need only? If no,wat other medicine i can use to q passage of food,
to get good bowels etc

You can take levosulpiride when you have symptoms.

Detailed Answer:
Hi,
If the symptom is only occasional and not bothersome, you can ignore it.
Do not eat full stomach. Instead, eat smaller meals.
You can take a low fiber diet. Try to avoid fatty food/oily food.
When you have symptoms you can take a short course of levosulpiride.
( T.Lesuride OD 75 mg 1-0- 0 half an hour before breakfast).
