default
Posted on Fri, 24 Jun 2016
Question: My dad been treated with alcoholism and depression since last one year.

A month ago he was taking
Stablon 12.5,baclofen 10 ,amizep h 12.5, three times and stresnil 0.5 at night.
But due to over sedation stresnil and 2 tablet of amizep and 1 of other two were withdrawn.

Other things being fine, but his old irritating behaviour is coming back.

Like replying in irritation on asking for food or anything normal.

Want to remain alone and mostly sleeping

If mom goes in his room, he said..get out of home.

What is this behaviour and what are the solutions.

Brief Answer:
Escitalopram to be started at night time

Detailed Answer:
Dear XXXXXXX
Thanks for positing at Healthcaremagic.

I am glad to know that he is currently out of delirium/confusion which was limiting his activity to a significant level.

Now I think we need to give some other medicines helpful for his irritation while not causing any side effects lime amizep or stressnil.

I feel adding 10 mg of escitalopram will be right choice for him considering the past history of side effects and increasing irritability over last two to four weeks. Since escitalopram is prescription only drug you need to visit your doctor to get prescription for same to buy it from medical store.

I hope this helps you further.
Thanks and regards.

thanks for your answer and remembering the case.

It's about baclofen. Now it's been many months he has stopped drinking. And I don't feel he has any intention in future.

So is baclofen still needed? Or we can stop it.

Brief Answer:
I think it need to be continued

Detailed Answer:
Dear XXXXXXX
Thanks for reverting back to me.

It is better to continue baclofen at this point considering his irritability and other ailments. He may not have any intention but unfortunately he is at risk of relapse. So it is better to continue at this point.

I hope this helps you.
If you more questions feel free to write back to me.
Thanks and regards.

Hello sir

I got an strange conditions today.
Since last few days my dad is very less interested in doing necessary things like eating food.
Neither he drinks water on his own nor want to eat food.

If mom tell him to eat or drink he gets angry.
Today he ran a bit to hit my mom when she asked for food.

And said, you have done this to me. You have made my mind like this..I don't know whether he meant it or just wanted to blame.

He also said- let me die if it comes.

I don't understand what is this..and what should I do

Brief Answer:
I hope you added escitalopram by now

Detailed Answer:
Dear XXXXXXX
Thanks for reverting back to me.

I like to assure you that what you call a strange situation is common thing after abstinence from alcohol. Many of the people blame themselves and others for their condition despite known fact that they abused alcohol through out their life.

These symptoms are clearly indicative of depression and I think the escitalopram should be started as soon as possible as it will help in irritability as well as depression. The presence of suicidal ideas is really important and patient should not be left alone.

It is also important to have personal consultation with treating psychiatrist as there are many conditions which can not be detected over online.

I hope this helps you.
Thanks and regards.

Brief Answer:
Use supportive approach

Detailed Answer:
Dear XXXXXXX
Thanks for reverting back to me.

I understand it is very difficult to calm down somebody who have significant irritability.

First of all we need supportive approach to him. Although he may criticize anybody but never to respond with same XXXXXXX I do not think he does deliberately but his medical condition makes him to behave in such a way. In addition you need to identify things which bring pleasant smile over his face. Try to present both pleasant things and food together. I understand he may again show irritability and may blame but be calm and respond calmly' ok, we are at fault. What need to be done now" or any similar statement depending on the condition or situation.

In case nothing makes him to eat it is indicative of severe depression and in that case I feel hospitalization is warranted.

I hope this again helps you to some extent.
In case you have more follow up questions please post a direct query rather than asking in same thread.
Thanks and regards
