Question: I am unmarried 24years old.i have irregular periods and sudden weight gain.i have hyperprolactinaemia and usg findings and tsh levels were normal.i am taking cabergoline 0.25 twice weekly and dronis 20 for past couple months.i am not having periods yet.i also have frequent vertigo.i have been suggested to do an mri.but mri findings will not change the treatment.so anyway how long should I continue the treatment and how should I take dronis pills?

Welcome to HCM,

I can understand your concerns,

As you told that you have irregular periods and weight gain. Prolactin level is high.and you are on treatment.

I advice that you should take tablet cabergolin for 3 months and should go for prolactin level testing. If it does not come in normal range, you can take cabergolin for next 3 months.

How much it is high?

I suggest that you should not take tablet dronis as it has side effects. Your periods will become normal once your prolactin level will reach to normal level.

Vertigo can be because of enlargement of pituitary gland in brain or side effect of drug cabergolin.

MRI was advised becase in case of great enlargement of pituitary gland , surgery is advised.

My prolactin level is currently 46.22ng/ml nd after peg treatment it is 27.18.ng/ml.my tsh levels are4.47microIU/ml.in 2011 I had same problems and I had taken a course of cabergolin for 3months as advised by the doctor.then my menstrual cycles normalised and I started losing weight.again since march 2015 my menstrual cycles became irregular and after visiting my doctor in july, my doctor adviced me to lose weight and take cabergolin and dronis.i do moderate execise but weight loss is quite difficult for me.anyway my question is should I do mri with or without contrast.i am slightly apprehensive about side effects of the contrast.so if I have pituitary adenoma then wil it be detected in non contrast mri.

Really sorry for the late response.

Its unfortunate that your prolactin levels keep rising.

Well, you would have to take cabergoline.

Coming to your question, In my opinion, contrast MRI is better especially to detect microadenoma of pituitary. It can clearly delineate the margins.

I understand your aporehension. But contrast MRI performed with trained hands is safe and there should be no major side effect.
