What Causes Irregular Menstrual Bleeding While On Progesterone?

Question: Hello,
I am a 29 yr old female with pcos. I am having a hard time becoming pregnant. I have been pregnant once about five year's ago but had a miscarriage at 12 weeks. I am seeing a fertility doctor who has said my ultrasounds and lab work is normal other than a vitamin d deficiency and a1c's at 5.8.
I rarely have a normal cycle. In October I was prescribed nine progesterone pills. I took three and had a normal cycle. In November I had bleeding from November 10th to Nov 20th(medium to heavy flow, I did not use progesterone) and then again from November 27 to December 1st( light flow) is this normal? Could this mean that my body is preparing for pregnancy? When would my fertile days be? Could this mean that I probably will not get pregnant? What could be causing this amount of bleeding?

I have read you query and I understand your concerns.

Following is my reply:

1) Vitamin D deficiency is not a contributing factor for infertility and better to ignore it as far as infertility is considered.

2) Your irregular cycles are due to PCOS.

3) Irregular flow was because you took progesterone pills for withdrawal. Another reason could be overweight ( Please let me know your height and weight). This does not mean your body is preparing for pregnancy. This is not good development to have such bleeding episodes.

4) You can get pregnant provided you seek right treatment from right fertility specialist.

5) Please reduce weight if you are obese or overweight which will regularise your cycles and also help in getting pregnant.

6) Please see an IVF specialist in your town so that your treatment can be started by giving you fertility tablets and doing regular follicular growth scan to study egg growth. It is the only way to predict fertile days in irregular cycle.

7) Your husband's semen analysis needs to be done meanwhile.

8) You need to get an HSG ( Hysterosalpingogram) test done to assess the tubal patency.
