Question: Hello Doctor

My name is XXXX XXXXXXX and i am 26 years old. For the past 2 months i had experienced lot of painful symptoms which are as follows

1)Irregular heartbeat
2)confusion
3)sudden tachycardia
4)blurred vision
5)salivation
6)dizziness and fainting
7)burning sensation in the stomach
8)constipation
9)anxiety
10)depression

I consulted several doctors, after which one of them apparently traced the reasons which caused the above underlying symptoms in my body. After evaluating the medical reports following conditions were reported

1)insufficient vitamin D
2)low sodium and calcium in BLOOD (ABG TEST)
3)Low Parathyroid hormone (PTH Test)
4)hy pylori bacterial infection (H Pylori Test)
5)low blood volume
6)low body fluid
7)Other reports like (CBC, Thyroid Profile, Lipid Profile, Sodium, Potassium, Calcium levels in bones, Creatnine serum, Urine albumin, urine creatinine and HBSAG were reported FINE)

After diagnosis He put me on following medicines for 1 month course

1)Pantocid HP for 14 days after which 40 mg pantaprazole for 14 days on empty stomach
2)cholecalciferol 2000 iu tablets
3)rejulong od (methylcobalamin, alpha lopoic acid, thaimine, pyridoxine tablets)
4)cadbeta xr
5)etizep 0.25mg
6)folvite tablets 5mg

I started taking these medicines on 17th November. While i do feel better but even today i am experiencing some weird symptoms like

1)tachycardia immediately after eating OR immediately after stading up
2)confusion, restlessness, dizziness, nausea and irregular heartbeat
3)involuntary MILD shaking of my hands legs fingers

These symptoms are haunting me day and night and i really don't know how long will it take to become normal again. I have some queries for the doctor
1)I would like to know whether the medicines are working or not. Its been 10 days since i started my medicines but still i encounter irregular heartbeats.
2)I feel that my doctor has not prescribed any medicine to increase sodium, calcium levels in blood and also the total blood volume. So considering the medications which i am taking right now please clarify whether i am right or wrong.
3)Also please let me know if i can adopt any lifestyle changes or consume ANY natural foods that can aid in quick recovery
4)Since i have low sodium levels in blood please tell me if i have to consume salts and water in moderate amounts or should i increase the consumption.
5)Since my hands fingers sometimes shake involuntarily,i feel that i may have Parkinson disease. Please clarify this point too
6)Also please tell me will my body become normal again. Right now my body is behaving strangely which makes it hard for me to tolerate. So please tell me how long will it take for me to become normal again.

Thank You

Hi and thank you for asking!
I read your query and understood your concerns.

First of all I would like to check by myself all your test results. Can you please upload all your test results for me?

Now I would like to answer to your query one by one:
1. Since most of your symptoms seems to be better after 10 days of treatment, it seems that the treatment is working for you. However it needs 1-2 months of treatment to evidence a full recovery of symptoms.

2. There's no need to prescribe medication to increase your sodium. You should just increase the salt intake (eat more salty food). Your doctor has prescribed hormones and vitamins which all together will regulate the calcium blood levels.

3. You should live a healthy lifestyle and diet which should consists as follows:
- eat a healthy diet reach in fresh fruits and vegetables, poor in diary products
- avoid alcohol, caffeine and energy drinks
- avoid consuming beens, cabbage, lettuce etc.
- Quit smoking
- avoid unnecessary stress
- Increase physical activity
- Take regularly chamomile tea

4. You should eat salty foods to regulate sodium balance.

5. The shaking hands most likely are related to hormonal and electrolytes imbalances then with Parkinson disease. Hormonal and electrolytes imbalances together with stress and anxiety should be blamed for numbness and shaking hands.

6. You should wait for 1-2 months and then everything will be back to normality. This should be confirmed by laboratory data (that should be done after 8weeks).

Hope this will be of help!
Wish you health!

Hello Doctor

Thanks for your quick reply. I have lot of doubts in mind

1)Please tell me about h pylori infection. Right now i am taking the medicines but i feel constipated. Can u explain the reason for the constipation?

2)Since my blood volume is low, will it increase by its own because my doctor has given me any medicine to increase the blood volume?

3)Since my sodium level is low.. what do i need to do. shall i decrease my fluid intake and increase the intake of salt?

4)You spoke about diet poor in dairy products. which means i should minimize its consumption, may i know why you said so?

PLEASE PROVIDE SATISFACTORY ANSWERS.

Thanks

Detailed Answer:
Hi back,

1. H. Pylori infection is responsible for stomach ache, acidity and gastritis. This all together affect intestinal motility causing constipation. Moreover, in a few cases constipation may be a side effects of Pantocid (Pantoprasol).

2. If your blood volume is slightly low there's no need for any special treatment. With the treatment you are taking and with dietary changes (increase salt and fluid intakes) your blood volume will be within norm after 2 months.

3. As i said above you should only increase your salt and fluid intake. This will increase your blood volume and sodium levels.

4. Milk and diary products, worsen stomach acidity and gastritis, worsening stomach ache and constipation. This is why I recommended to reduce milk and diary consumption.

Hello Doctor

Thanks for the reply. I am suffering from severe constipation since 2 days. I have been eating foods rich in both soluble and insoluble fiber, drinking plenty of water and taking medicines for H pylori infection but still i dont feel the urge to access the toilet. I wanted to know is this constipation because of the H Pylori infection which may have come back or maybe the triple therapy medicine Pantocid HP( Clarithromycin, pantaprazole and amoxicillin) has failed to fight the bacterial infection even after finishing the 14 days course. Please let me know why am i feeling so much constipated.. 2)Please prescribe a safe laxative which can help me relieve this constipation.

Thanks Doctor

Detailed Answer:
Hi back,

The constipation will be relieved within a couple of days with dietary changes (you have already done) and with adequate treatment. If you were my patients I would add the following suggestions:
- Use extra virgin olive oil 1 table spoon three times a day
- Use Metamucil OTC 1-2 tea spoon three times a day
- Use Bisacodyl 1 tablets three times a day.

These will help you with constipation. However, if the constipation will not be relieved after two days you should do a rectal enema and then continue with dietary changes and treatment.

The constipation isn't related to H. Pylori infection because after two weeks of treatment H. Pylori is eradicated from your body. However gastritis and stomach acidity witch are the cause of constipation, need 1-2 months of treatment with Pantoprasole to be completely healed.

Hope this will be of help!
Wish you health!

Hello Doctor

Thanks for the response. The medicines which you prescribed for constipation has been a savior. Also i am on other medicines and recovering well from the illness but there is one thing which concerns me, sometimes my body shakes abruptly involuntarily. Its not the entire body but just my limbs. After which there are episodes of sudden increase in heart beat which climbs upto somewhere but below 100 BPM.This causes uneasiness and restlessness in me. One more thing i wanted to express is i have no heart disease. i have undergone ECG ECHO and HOLTER TESTS which were reported absolutely normal. Therefore i wanted to know what is causing this problem. Are these episodes akin to seizures due to electrolyte disturbances in the body. Will my body become stable again? This is my concern

Thanks

Detailed Answer:
Hi back,
Sorry for my delayed response. It's a great pleasure to know that my advice were of great help.

Your symptoms may be a consequence of electrolyte and hormone imbalance. This may trigger your symptoms which increase your concerns and anxiety causing tachycardia and worsening your symptoms and overall health.

My opinion is that most of your symptoms will be normalized after two months of treatment. In order to completely relieve your symptoms it's essential to reduce your stress and anxiety level. Please keep calm and have faith in the efficacy of the treatment.
Everything will be normal again within 2 months.

Wish you health!
