Brief Answer:
May be musculoskeletal or esophageal, probably needs no specific action.

Detailed Answer:
Hello,
This is very atypical for cardiac, so less likely to be cardiac. The probable causes for it are, either it is a musculoskeletal pain, cramps or streching of the intercostal muscles, or it can be esophageal spasm. Due you had local pain on pressing the area, it yes it suggest local musculoskeletal. Do u have associated nausea, bloating, burping, chest burning, if yes then it points toward reflux disease and gastritis, was there any association with breathing.
Overall doesnt looks like a matter of worry. Can try local diclofenac gel if any local pain or tenderness is there, Tab Pan DSR 40 mg before breakfast once a day.
Overall, you are very low risk for cardiac illness beacause you dont have any risk factor other than smoking and your adequate work up disease ia already done.
