What Causes Intermenstrual Bleeding?

Question: Hi Doctor,
I am 39 yrs old and I started having this mid cycle bleeding since last month. Last month it happened at my 12th cycle day and was very little amount of red blood mixed with transparent discharge. It happened once when I cleaned myself with toilet paper after nr 1 and stopped, never happened again. This month same thing happened in my 10th cycle day and stopped after cleaning toilet paper just like before. But after 3-4 days, today, it happened again. A very small amount of blood and brownish. I have slight pain in my lower abdomen, more like a pressure sensation with burning.
My periods are very regular (26-27 days) with slightly increased red bleeding since 2-3 months. But still finishing clean in 5th cycle day. I have a gyn visit in 4 days but I am worried that this second short bleeding this month can indicate a serious problem like cancer? Since I heard one can ovulate only once in a month therefore bleeding occurs once. I would love to ask if I should be worried. I have my PAP smear every 3 years, did this year's test 2 weeks ago and waiting for results. My previous results were alright. Also, both my mom and grandma had their menopause (finished) when they were 42.

I have gone through your history and understand your concerns.
There are so many causes of intermenstrual bleeding ( bleeding between period)other than cancer but before giving my opinion I would like to ask a few questions from you.
Are you on birth control pills?
Are you suffering from any disease or taking any medication?
Have you done your thyroid profile?
Please specify the site of pain?
Any type of vaginal discharge,itching etc?
Are you married and sexually active and having kids?
Burning in urination or any other symptoms?

Hello Dr. XXXXXXX Thank you for your reply!

The answers by order;
- No, I am not on birth control pills now but I have used them before for around 2,5 years.
- I am not currently suffering from a disease but I am taking antidepressant (Sertraline) because of my anxiety problem.
- I have done a blood test including all thyroid tests around 4 months ago and my results were all fine.
- The sensation I am having is a little pressure feeling and slight burning feeling in lower abdomen, right in middle where bladder is. Sometimes I have pain in lower back, a dull ache feeling but not too often and it comes and goes. When I lie down and/or sleeping I have no pain. I don't wake up with pain. I use to have back pain in same area, it's nothing new.
- I had a yellow thick discharge right before ovulation this month but it stopped and became the regular transparent discharge. No itching. I also sometimes have a very slight light brown tint in discharge but none of my discharges are continuous.
- I am married with no kids, I am sexually active with my husband naturally. I am pretty sure he is also only having sexual reltionship with me also I was his first. I am pretty sure about this. My previous sexual relationship was my first husband, I can't say I am sure that he wasn't with others. But it was 10 years ago. I didn't have any relationship with other people.
- I am experiencing slight burning with urination sometimes, especially after I am done and go back to my room. But no urgency of urination or fever. I have had UTIs before in my life. In any case the nurse I talked on telephone asked a dr to write me light antibiotics specially aiming UTI. I am currently using it now.

I also got a Gyn check up with transvaginal ultrasound around 10 months ago and no problem found. But back then I didn't have any mid cycle spotting problem.

Hello dear,
Sorry for the long delay in my response.
I have gone through your history and in my opinion hormonal imbalance is a cause of your problem and maybe resolve in 2-3 cycles by its own.
But you have to reduce your anxiety because stress cause hormonal imbalance.
Your abdominal pain and sensation maybe due to UTI( maybe renal stone) and it can also cause mid cycle bleeding.
You should drink a lot of water,keep the area dry and clean,take antibiotics( according to culture and sensitivity) and never hold your urine.
You should consult a Gynecologist if problem persists after 2-3 cycles and get yourself examined and investigation done to rule out the cause.

I am trying to drink lots of water and trying to get use to it as a lifestyle habit.

I will see a gynecolohist on thursday anyways but I am relieved that it is most likely hormone related, than a very serious thing like cancer.
