default
Posted on Thu, 27 Apr 2017
Question: when I awaken from sleep over night or dose off for just 5 minutes and awaken I feel deep depression every day. I recently took a sleep study test that only showed only mild sleep apena. My diet and exercise are moderate 30 minutes a day to help treat the pain associated with lower lumbar disease. I have excellent stress management skills, over the last 5 years if have developed sleeping problem like insomnia. I'm a 58 year old male no with no drug or alcohol use but I do smoke cigarettes. The severe depression I described above will last for about about 2 hours, Through out the day not as server but with out reason instantly during good activity or normal activity. best I can describe it like a wave of bad energy just takes over.
through out the day and I feel helpless of any control. The one thing that I feel that seems to be common is the deep depression is connected to a physical activity when I awaken from sleep. and what can cause instant mood change with out warning. I do not have a list of the medications I've been on that suggested they were for depression but I'll say over 30 some with combinations with one another.

Thank You,
MDE

Detailed Answer:
Hi, thanks for asking from HCM for your health related doubts

I can understand your concern. Though its difficult to explain exact cause of symptoms you're having but these are related to depressive disorders only. You have worsening of depressive symptoms on waking up from sleep. This could be explained by psychophysiology of depressive disorders.

In most of individuals in depression REM or Rapid Eye Movement sleep increases. Infact reduced REM latency is one of common features of endogenous depression. Increased REM sleep worsens the depressive symptoms. Awakening in REM is resulting in worsening of mood most probably.

Most of antidepressants work by reducing REM duration. Use of Amitriptyline, Paroxetine which is a SSRI drug can help to improve depression symptoms and will decrease the duration of REM sleep and you will see improvement of your symptoms.

Your sleep study revealed sleep apnoea but did they comment on REM duration in sleep?

Thanks. Take care
