Question: I have a psychiatrist who has significantly increased my Seroquel from 50-75 mg to 125-200mg HS (for severe chronic insomnia ) I am noticing that my Blood Sugars are running higher and I'm gaining weight ( which I don't need). I'm TY 2 DM. Is there any thing I can do to help these 2 issues? I usually walk 2-3 miles most days up hill and down hill and in WY the wind.

Antipsychotics, which could increase blood sugar level includes olanzapine, risperidone, clozapine and quetiapine, so better to avoid them. Less chances are with haloperidol, aripiprazole and ziprasidone, but neither of them have much sedative effect. Antidepressant with high chance of diabetes is mirtazapine, so better to avoid it. You have been taking seroquel for sleep, in that case, you can also try either any benzodiazepine or another sedative antidepressant like trazodone.

If you were my patient, i would have suggested trazodone, rest you can discuss with you psychiatrist.

trazodone doesn't work for me. It gives me significant palpitations at around 100-110bpm, which also makes me anxious. Also can I become tolerant to seroquel?

Hi

Yes, gradually you can become tolerant to seroquel. Tolerance is not only possible with seroquel, but also with other drugs. In think, in your case, you can try either carry on with seroquel or shift to benzodiazepine. Chances of getting tolerance with benzodiazine is even higher than seroquel. You can use drugs in combination like benzodiazepine with melotonin or seroquel with benzodiazepine etc. That would keep the dose at minimum level. Try to avoid the dose when you can get sleep without them. That would decrease the chances of tolerance. You can also try relaxation technique like JPMR and avoid any stimulant before sleep.
