What Causes High WBC Count During Pregnancy?

Question: Hi, My wife was in her late 8th Month pregnancy and she got hospitalized for Fever, Weekness & Gastrics. She had been having fever on and off upto 10.5 F and was admitted for a week. After examination, the WBC count was 18000 and she has lose stools too. So the Doctor advised her Magnus Forte 1.5 for 7 days. After 5 days of stay at hospital, she was stable and we got discharged. Now yesterday afternoon (She is in her 9th month of pregnancy), she got back fever and it went till 100.6, as advised by the doc we gave her a Dolo 650. However the feverish feeling was coming and going in the evening. Doctor advised to examine routine Urine, blood CBC and stool. Below is the analysis report. WBC - 14360 Platelet Count - 472000 WBC/Pus Cell - 2-4/hpf Epethelial Cell - 10-15 /hpf Pls advice as to what sort of care should we take and what is the reason for the above readings. Thanks XXXX

If she developed fever again , it can be one of the two things: 1. A new infection. 2. The old infection is still persisting. Since, last time she was just treated and not diagnosed, we might be missing some thing. Het WBC Count is high, means she has infection. I need to know following : 1. What diagnosis was written on discharge sheet? Attach it if you have. 2. Fever is high grade/ low grade? 3. Any chills and rigors associated? 4. Any Cough with expectoration? 5. Any weight loss? 6. Any bleeding/ jaundice? 7. Any burning micturition? Reply with these and attach all the previous investigations, so that i can study them. This is my personal opinion based on details available here. Hello Dr XXXXXXX Thanks for the quick response. Pls find the requested details. I need to know following : 1. What diagnosis was written on discharge sheet? Attach it if you have. -She was advised to complete the Antibiotic (7 days course), Take gastric, diarrohea and fever medicines on SOS basis. Avoid Milk food and take rest. 2. Fever is high grade/ low grade? -Fever has been btw 99 to 100.6 always 3. Any chills and rigors associated? Yes, she gets chills before the fever comes up. 4. Any Cough with expectoration? No 5. Any weight loss? She was 58 Kgs and during pregnancy, it went upto 66 kgs. Now she was lost 3 to 4 Kgs and her current weight is 62.5 Kgs 6. Any bleeding/ jaundice? No 7. Any burning micturition? No Pls advice. regards XXXX

Brief Answer: She should be investigated. Detailed Answer: Dear XXXXXXX I appreciate your well explained reply to my questions. I am just concerned why a diagnosis was not given to you on your discharge by your doctor. As, with qualified doctor or hospital would give. You have not shared with me her all investigations of that period of admission. I would like to get following investigations : 1. ESR 2. LFT and KFT 3. Urine and Blood Culture 4. Widal test 5. TORCH Test 6. Xray Chest (with abdominal shiled) - This can be avoided if her chest examination by the doctor is absolutely normal. I just want to rule out any Pneumonitis or Tuberculosis. If you have her reports, send them to me. Regarding treatment part, get her these investigations and give her Dolo 650 as advised by your doctor. Maintain her hydration. But since her WBC counts are high, she would require antibiotic therapy. If you have any further query, please ask me.
