default
Posted on Tue, 6 Oct 2015
Question: i am on lisinapril for high PB.25mg a day. PB has been under control; about130/75 for 6 months.I am a healthy 85. Yesterday the PB spiked to 175 /95 and has more or less stayed there for 24 hours. No change in life style, or other meds.First ever colonoscopy on Tuesday next for diverticulosis, and Im a bit up tight over it. Took 1mg XANAX this AM..no change in PB. Only one visit to a hospital in 85years, 6 months ago, for stage 2 diverticulosis, cured with meds...no surgery..Is this a hospital visit? What could cause a sudden spike i PB?

any chance cutting out all vitamins...fish oil,on a day, etc etc could cause the elevated PB? I took a lot of them.....

Detailed Answer:
Hello!

Thank you for asking on HCM!

I understand your concern and would explain that the fact that you had a spike of high blood pressure doesn't mean that your hypertension is persistently uncontrolled and your antihypertensive therapy isn't correct.

It is just any triggering factor that has transiently adversely affected this balance (between hypertension and beneficial therapy effects).

I believe that your recent health conditions (your hospital visit and especially the uncomfortable diagnostic procedure such as colonoscopy) has induced this blood pressure changes.

High blood pressure bouts during in hospital medical check up are well known under the definition of White coat syndrome, which is explained by the psychological stress imposed by the medical contact.

These episodes of high blood pressure are generally not harmful and are quickly resolved spontaneously after a period of time.

From the other hand the colonoscopy procedure produces a lot of distress, discomfort and anxiety.

I would recommend to closely monitor your blood pressure for a couple of days (several blood pressure measurements daily) and if those high BP values persist, you can temporally increase the dose of lisinopril up to 40 mg daily, and when blood pressure values decrease, you can turn to your usual anti-hypertensive dose regimen.

Meanwhile you should avoid caffeine intake, psychological or physical stress, salty and spicy food and at the end the most important issue is to relieve your anxiety.

Just remember that diverticulosis is quite a manageable disorder. So relax and do not worry about it.

Hope to have been helpful!

I am at your disposal for any other questions, whenever you need!

Best regards,

Dr. Iliri
