Brief Answer:
Surgery might be necessary to set you free from the symptoms.

Detailed Answer:
Hello!

Thank you for the query.

How long have you been taking Omeprazole? Have you had gastroscopy done?

Generally, if Omeprazole in such dosage (40 is a high dosage) does not help, proper diagnostic and treatment is necessary.
First of all, you should have gastroscopy done (if its not done yet). Depending of the findings (you may have Helicobacter Pylori infection), proper treatment can be ordered (antibiotics).
If this wont help, or gastrsoscopy will be clear, you should have a test called 24 hours pH monitoring. Depending of this tests result, you may need laparoscopic surgery called fundoplication.

You should also avoid coffee, fatty foods, spicy foods, alcohol, chocolate, orange juices.

Hope this helps. Feel free to ask further questions.
Regards.

I have been taking omeprazole for around 5 years. I have not had a gastroscopy. I am a former smoker with moderate COPD.These symptoms just started about 2 weeks ago.

Brief Answer:
Gastroscopy has to be done.

Detailed Answer:
Than you should have gastroscopy as soon as possible. Smoking makes you prone to esophageal and stomach cancer. Gastroscopy should rule it out.
