Question: Hi there. I am a 43 year old healthy female. In January I got a horrible fuzzy bad sensation in my right ear. The doc prescribed an antifungal ear drop which I used, when the pain hadn't cleared he prescribed a second bottle of the ear drop (cannot remember the name). That didn't work and I went to see a different doc who said I had a bad ear infection and prescribed ammoxicillan. That didn't work and by this stage the pain was really bad so she took an ear swab and prescribed a different oral antibiotic (again I cannot remember the name). I finished the course but the pain was still bad so I went back to the GP again and she had a look and said the ear infection was still there and bad and prescribed gentisone ear drops. I took these for a week and the pain eased a bit although these drops left my ear very itchy. I decided at this point to give it a few weeks to see if my ear would heal up by itself. I didn't so I went back to the GP (mid March by now) and she prescribed another bottle of Gentisone as this was the only thing that seemed to have any impact on the infection to this point. I have finished this second bottle of Gentisone ear drops and by ear is worse than ever. Not only is it painful from the infection but it is now so itchy its driving me crazy. I cannot sleep without pain killers and it leaving me irritable and crabby with two small children in the house. I have not been able to use the pool at my gym (I joined in January hoping to get fit and even doing any kind of fitness is leaving me feel dizzy and sick). Please help as I am at my wits end at this stage and I'm also so scared that all of this is doing long term damage to my ear and possibly leaving me open to re-occurring problems in the future. Please get your ear examined by an ENT to rule out any perforation of tympanic membrane (TM). If so, then it needs to be treated with antibiotics orally.
If there is no perforation and there are no signs of middle ear infection (which can be seen by examining TM), the problem can be thought of related to infection of the external auditory canal.
If it is infection of the outer surface of TM and external auditory canal, ear drops with a combination of antibiotic with steroids (plus minus antifungal depending upon findings) should be used and this will take care of the itching part also and help in preventing repeated trauma to the ear because of itching.
If your ENT considers the infection to be severe, an ear swab and culture sensitivity and then specific antibiotics will help.
