Question: Hi Doctor,I am facing stomach ache every often. every time i eat something i have to excrete it. nowadays stomach has also started hurting.. a bit.. i had a complain of gastric. and i took medication for that a month back.. it seems it has reduced. but stomach is lil bit bulged which wasnt the case before.. PLease advise. Thanks, XXXX

Read and understood your history and concerns.

To recapitulate:
Female/ 28 - has gastric and took medicine for that a month back - seemed that it reduced - bulging of stomach - now stomach ache very often - have to excrete every time you eat something...

With the history your have provided, I think of the following:
- The problem started with intestinal infection which was not not treated completely hence the sub-acute attack continues. There is a possibility of some inflammatory pathology in the abdomen.

I would advise you the following:
-First of all, get all the relevant blood tests, including Widal for typhoid, thyroid and liver function, blood sugar done.
-Stool test for 3 consecutive days.
-Ultrasonography of the abdomen and X-ray of the abdomen in standing position.
-Get started on suitable antibiotic, metronidazole for 5 days. Further course as per the reports.
- Pre and probiotics, PPI like Omeprazole, Activated charcoal tablet to assist in getting better.
- If there is complete relief, further treatment as per the reports.
- If there is less than relief then further tests of CT scan of the abdomen Upper GI endoscopy, Colonoscopy as required to find out any other problem in the abdomen. If found to get it treated.

If all the above investigations are normal and the problem still continues in spite of proper treatment, you may be treated as a case of IBS- irritable bowel syndrome.
