Fever associated with lower abdominal pain requires a proper clinical appreciated. The site of the pain in the lower abdomen is of utmost importance. It would also be better to know the duration, intensity, associated symptoms like nausea, vomiting and stool frequency. In her case especially, it is of utmost importance to be sure that this is not an abdomen that would require surgery (pain, fever, constipation). It might be of interest to know if she was constipated before having the abdominal pain or not, and also for how long she has been constipated.

Blood tests like a complete blood count shall be needed. Raised white blood cell counts and cRP counts shall be suggestive of a bacterial infection. A plain film of the abdomen could be useful in determining if there is an intestinal occlusion and also giving an idea of the probable site. An abdominal ultrasound could be useful in indicating any swelling, mass or cyst. A stool examination, culture and antibiogramme could be useful in identifying the germ and probable best of choice of antibiotic that could be used in treatment.

Prescription of these tests however shall depend on a good clinical review. Causes of such symptoms could range from simple gastrointestinal infections like typhoid fever, amoebiasis to more serious conditions like appendicitis.

I strongly suggest she sees a Dr. Her sinus infection requires a proper review by her ENT specialist. Antibiotics could be sufficient, and surgical drainage might be needed at some point in time in order to prevent complications. A review and appropriate X rays and other paraclinical tests would be of help in case the clinical status after examination by her doctors warrants these.
