What Causes Fever And Yellow Dusty Cough After TB Treatment?

Question: hi,

I am XXXXXXX , A software engineer by profession. I had jaundice in 2003-2004 and after some test and home remedies i was fine. I had a periodic fever that time, I consulted many physician and gastrologist across India . My USG reports was showing mild splenomegaly(12.3cm). They did colonoscopy several times and prescribed some medication to subside fever and after two three normal days again same story repeated. In 2010, I had same fever , this time lasted for long and i had to admit. Doc did a CT scan where spleanomegly and Masentro lymphadenopathy was shown. Typhoid +, mountex test +, quantiferron gold(for TB) positive and they diagnosed TB(Koch's abdomen) and started medication. After 2 months of medication i had fever again and i got admited to other hospital and they did MRI but no lymph node enlargement found. But still they advised me to continue TB medicine. Then i had a follow up in CMC vellore, and ther studied all reports and did duodenitiis biopsy while colonoscopy. findings were-(minimal chronic duodenitis, D2 mucosal biopsy minimal chronic gastritis with mild atropgy, antral and incisural mucosal biopsy). and they diagnosed old abdomen tuberculosis, irritable bowel syndrome, entameba dispar infection and gave one medicine only. Since Dec 2010 when i visited CMC, i have been doing very well(no ferevr, no weakness) till March 2014 when i felt severe weakness. consulted a physician with all history, he advised CBC, LFT. LFT was normal but CBC had low RBC n platlets. After some medications and repeated CBC test, RBC and platlet was further decreased with slight changes in other differential components as well. iron studeis were normal, dengue normal, peripheral smear normal, b12 normal, D3 vitamine normal but no chnages in symptoms. I consulted with a gastro with all history and he did a fibroscan ( a kind of liver biopsy-non inhesive) and said my spleen is not enlarged coz of liver. Liver is absolutely fine. Prescribed me some multivitamine but no relief. Then i went to a XXXXXXX physican with much experiance and he said my spleen will not shrink, we have to rule out the symptoms and referred to a neuro physican who gave some tests including NCV 4 limbs , RA factor, ANA. RA , ANA is negative, rest all reports are yet to come. no weight loss. only weakness and especially in afternoon and that too down the knees and anckles. no improvements on CBC. one week back in one lab platlet was 94000 and after two days in another lab it was 104000. Sometimes GIANT PLATELETS SEEN in my sample. Can u pls guide me. I am very tense now a days beacuse of this..
CURRENT SYMPTOMS-
General weakness
Specialized weakness down the knees and anckles.
no weight loss but weakness can be seen by any people. Cloths seems loose. I look like getting slim.
Sometimes no feeling of hungry and some time always feel hungry.
abdomen pain sometime( i think natural it is) after eating
lowered tone while talking in afternoon. Easily recognizable by fellow people.
I can't stay stand for more tha 5 minutes. I feel like i should sit or lay down. very uneasiness in knees and ankles and down the anckles,
No joint pain
No problem in walking. I have problem when i am standing.
One thing to notice I always see yellow/dusty cough.
mild chest pain(right side )- not always. in certain mode only. May be because i used to go gym before these symptoms and stopped now a days.

Thanks fir your query and an elucidate history.

It all started in 2003 and as per the reports you have noted the problem looked like Koch's abdomen. The disappearance of the Lymph nodes is again suggestive therapeutic effects of Anti-tubercular treatment.

Your getting fever again can have another reason/s as the body resistance reduces drastically due to tuberculosis. The most probable tablet you had received must have been metronidazole or so..

Good to know that you kept well from 2010 to 2014.

And now you have weakness as a major complaint associated to the problems you have mentioned; yellow/ dusty cough.

Let us talk and think about the basics. I would advise you to go for simple tests::
Sputum for - gram staining, culture and sensitivity and AFB (TB) staining. Widal for typhoid, Malaria by slide and antigen. Ultrasonography to see for liver and other affections of the abdomen. Urine culture and sensitivity.
X-ray Chest ;

This time too you can take Doxycyclin, Metronidazole, antimalarial treatment under the guidance and supervision only.
Take this 7 days , get the reports and clinical review by your treating Doctor. Add Protein Power, take Ginseng with Jingko-biloba, Continue CalMagD ...

I hope there is not much of a mental stress you have..Re-read the present complaints, all are suggestive of an asthenia. The treatment suggested is to be discussed with your Doctor as you may a prescription and follow-up.

I have a feeling this treatment will help you a lot. Plus add on drinking plenty of oral fluids, Take full bed rest if possible, change the atmosphere around you.

I would try to give a brief history of all you have suggested:"

While treatment of tuberculosis, i think AFB sputum test was done approx 50 times and all test were negative. I Dont have any fever since 2010. Last ultrasound was done on Sept 2013(due to some abdomen pain)because having seen my history, i get every abnormality checked and take it serious. Urine culture was last done on Sept 2013. everything was fine except mild spleanomagely in ultrasound. Physicans says might be I had untreated prolonged malaria and thats why my spleen is enlarged but it is almost constant so let it be..it wont subside and due to this spleanomagely you will often have low blood count and platlet. I had CBC done 5 times in last two months and I am attching all CBC reports here. Also, I have gained 12 kg weight since 2010 and my doctor frineds says u cant have TB else u would not have gained weight. Even last X-ray done in 2013 was also fine.

I would request you to please go through attached reports and guide me further.I have attached many relevant reports and upload was successful but i can't see all of them after refreshing the page.(is there any problem or u can see all attachments??)

RA ANA and latest CBC done on 28th April is also attached.Pls let me know if you are unable to see all reports.

Is there any permanent treatment for enlarged spleen?? In homeo or Ayurveda??

Is there a chance of serious illness?? Now a days m very much tensed about health.

Thanks for all the relevant reports.

The spleen size of an adult on ultrasonography is 12 to 14 cm on an upper scale. I have reviewed all the reports- It was 12 cm in April 2005 and is within the range in all the reports till now. Because it is at the higher levels, it is reported as ''mild'' splenomegaly. I am not aware of treatment in Homeopathy or Ayurveda for splenomegaly and I would suggest not to go for it.

Secondly- if the blood cells were to get sequestrate more than the normal, the spleen would have grown too much by this time. So I do not personally think that there is any problem with the spleen.

I would rather worry about our blood counts. It is very true that the reports like you have been thoroughly researched and no reason is found. That may be the capacity of the bone marrow to produce this much only.

Hb, PCV, WBC, RBC, Platelets are at the lower limits. This may indicate a problem with the bone marrow. But definitely not a serious matter as they are touching the lower limits ( to repeat).

I would suggest you to consult a Hematologist for further clarification and suggestions and investigations as per His / Her Advise.

Once the Hematologist declares the status you will be stress-free.
