Question: Brief History
Right from childhood I have unintentionally developed a habit of negative thinking.
When I was in school I was weak in studies.I was unable to cope-up with changing environment.
I was always cornered by my classmates.
When I was in T.Y. suddenly I started feeling huge stress and fear in my mind about future and career.
It increased to such a level that while writing my hands used to shiver and I could not write properly.
When I completed my graduation and got a first job my father became very sick due to pneumonia and was quite serious. It created a good amount of pressure on my mind as I had to do job and take care of father. I always used to suffer from loneliness and restlessness.It built up a habit of negative thinking in my mind permanently.
In my recent job also in the beginning I was new with the work and was unaware of the system.
In the beginning of the current job I suffered badly because I was taking little more time to learn and understand the process & system.But instead of supporting me my boss and my colleagues used to pin point my very small and marginal mistakes and my boss submitted my C.R. containing negative remarks and I got two letters from our H.O. regarding the same.I was feeling very helpless at that time and thinking to leave the job.But after that I could cope-up with the system and process and now I don’t face any problems while doing work in the office.

So, my current problems are


1)     FEELING OF UNCERTAINITY ABOUT FUTURE.
2)     ALWAYS CLASHES IN MIND ABOUT TO DO OR NOT TO DO THINGS.
3)     GAP BETWEEN EXPECTATIONS AND ACTUAL SITUATION.
4)     ALWAYS FEELING OF STRESS,FEAR,RESTLESSNESS.
5)     ERUPTION OF PAST BAD MEMORIES IN MIND CREATING STRESS ON MIND.
6)     FEELING OF LONELINESS.
7)     HYPER NAGATIVITY.
8)     CLASHES IN MINDS.
9)     UNRELATED THOUGHTS
10)     PROBLEMS IN LEARNING NEW THINGS.
11)     RELUCTANT TO ACCEPT NEW THINGS.
12)     LITTLE BIT SOCIAL FOBIA.
13)     BUMPING UP OF PAINFUL FEELINGS.

Treatment with medications plus CBT

Detailed Answer:
Dear Mr. XXXXXXX

I read your query in great detail and do understand what you might be going through. You seem to have been suffering quite long, and I do appreciate that you have decided to come out and seek help.

From the situations and symptoms that you have mentioned, it seems that you might be suffering from a variant of depression, something called dysthymia. In addition, your depressive symptoms might have fluctuated throughout from little to very distressing. You might also have ingrained some of these symptoms as part of your personality.

At this stage, my suggestion will be that you seek a psychiatric consultation for your current situation. Had I been your treating psychiatrist, I would have offered you treatment with medications as well a form of psychotherapy termed cognitive behavior therapy (CBT).

The medications will include anti-depressants like escitalopram or sertraline. These medications are quite safe. However, they are slow to act and you will have to be patient about the response to the medicines. They will be continued for months and has to be continued even after symptoms have reduced.

CBT is a form of treatment where steps are taken to help a person change the way he thinks, correct the distortions that may be present in his thinking (like negativity), and also behavioral aspects which will help him relax more. Such treatment is usually done for 12-20 sessions, one or twice a week for 45-60 minutes. You can discuss regarding the same with the psychiatrist you would consult.

Dear XXXXXXX Sir,
Thanks for giving answer to my question.
In the above question I fogot to mention one thing that few years ago I had been consuming Ozepam 0.5 and was feeling very relaxed for that time.
But now the restrictions imposed to pharmacists to not allow medicines like this without prescription I am unable to get Ozepam 0.5.
So,what can I do in this situation? Would you please tell me name of other anti-depressants like Ozepam 0.5 which not requires precscription or would please arrange a prescription in my name mentioning the tablet Ozepam 0.5 as it creats a great relaxation in my mind.

Dear Mr. XXXXXXX

Thanks for following up. I can understand that you would be feeling very comfortable after taking ozepam. Ozeapam is the brand name for clonazepam which is benzodiazepine and helps in decreasing anxiety as well as improves sleep. However, it is a prescription drug which is why you are facing difficulty in procuring it without prescription.

As a psychiatrist however, I would advise you not to take clonazepam for long periods of time. It can lead to development of dependence plus it is not the treatment for your condition. It just helps in reducing anxiety for the time beig and when its effect gets over, there will be recurrence in anxiety. Gradually, you will also have to increase the dose of clonazepam as more dose will be required to produce the same effect. It will be difficult for you to stop it then as it will cause withdrawal symptoms when you will try to stop it which will lead to increase in anxiety.

We use clonazepam or any other benzodiazepine only for short periods and under close supervision. It will be better if you consult a psychiatrist in person. That way, you will can be started on proper treatment, preferably with one of the selective serotonin reuptake inhibitors like escitalopram or sertraline which will help you in controlling your disorder per say. Once your disease is under ontrol, you won't feel anxious or depressed.

Most of the psychotropics are available only on prescription until and unless someone sells it illegally to you. So, you will have to meet a psychiatrist in person for the same.
