Question: symptoms similar to low blood sugar... Type II Diabetic. current BS 189 on 50 units Lantus am and pm
took 8 units s/t insulin this am because reading 290. Now- Blood pressure 123/81 Pulse 144!

Perspiring, foggy, feel like might faint, headache, shaky


I FORGOT IN PREV MSG. I have been working 12 hour days mostly the last 2 months (self employed). Very tired, also care giver to my husband who had stroke in 2014 Oct.

My memory takes longer these days.... I took 50 Mg of Metoprolol ER Suc. about an hour ago. I normally take 25 Mg of it (not ER), but have been out for 2 days. going to pick it up later today and get back on it.

Take Glucose immediately.

Hi,

Noted your concern.

Your symptoms denote medical emergency.
If Home glucometer is available check blood glucose level immediately.
If blood glucose level is below 70 or Glucometer is not available take 15 gram of Glucose powder dissolved in water immediately. If Glucose powder is not available use home sugar.
After having this eat some food containing carbohydrate.

If blood sugar reading is normal go to ER immediately.

Do this immediately.
Afterward get back to me for further opinion.

My blood sugar readings have been high in am, before I took 50 units of Lantus.

Now at 1:02 pm, it is 119. I still feel symptoms similar to really low readings as in the 50's. Still shaky, perspiring vitals 1:14 pm, 126 78 104
Now hydrating with water.

Additional information: I am a caregiver to my husband ..who had a stroke in Oct 2014, not able to remember (short term) most things. I've been working long days at home sitting at computer/desk all day. Really exhausted and have
Fibromyalgia, with CFS - have to take Adderall 40 mg in am. to be able to get up and work every day. We both were taking Levemir Insulin until last week. BC/BS Advantage of AZ doesn't pay for it, so we had to switch to Lantus (per our temp. PCP) who is not a provider with BC/BS. The Lantus seems to work differently. We were waiting on new ID cards for the new PCP, and we did not get them. Called today, resending (person who took my request to change left part of our address off, so PO didn't deliver. New Doctor wouldn't make appts. for us until we could bring that card with us with their name on it. So all of this is the reason for seeking information as opposed to local care outside of the Ins. After my first contact I started feeling somewhat better. The shakiness has ceased, and the perspiring. Feeling much better now. Thank you for your response.

In some cases symptoms of hypoglycaemia develop even at relatively normal blood sugar level, especially in patient with long standing uncontrolled blood sugar levels.

Also dose of lantus is quite high.
I may suggest to monitor blood sugar level frequently with home glucometer as follows-
Monitor any 2 to 3 readings per day out of following:-
-     Fasting sugar levels
-     2 hours after breakfast
-     Before lunch.
-     2 hours after lunch.
-     Before dinner.
-     2 hours after dinner.
Addition readings when there are symptoms of hypoglycemia note down reading, timing, any changes in insulin dose, food or exercise pattern.

Depending upon this chart your doctor can help in appropriate management of blood sugar level and insulin doses.

Lantus and Levimir both are long acting insulin with different mode of action.

Action of Lantus may last for more hours as compared to Levimir hence may show better blood sugar control than levimir in many cases.
I personally use lantus more frequently than levimir in my practice.

If symptoms similar to that of hypoglycaemia develop even with normal or high blood sugar level, better to go ER for detailed evaluation.

I may suggest following blood tests -
- Complete Blood count.
- Urea, Creatinine, Electrolytes.
- Liver Function Test.
- HbA1c.
- Lipid Profile.
- Urine examination including urine albumin creatinine ratio.
