What Causes Fatigue, Stress And Weight Loss?

Question: I feel fatigued all the time. Even if I have less work, I feel stressed. I have feel I have no energy left in my body. I feel like I can do no exercise at all. I lost a lot of weight in last couple of months. I sleep a lot. I am still tired even after a 7/8 hours of sleeping. I am almost always yawning. Please suggest.

Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
Usually such generalized fatigue, stress, weight loss are due to anxiety or fibromyalgia.
However to state that we should first of all rule out issues like Anemia, Calcium and Vitamin D deficiency.
In case you snore in night, you might be having Obstructive sleep apnea, that can be one of the reason for the tiredness and yawning in the day time.
Ask your family members, if you snore excessively in night then you need to get a Sleep study done and to work for weight loss in case you are overweight.

I would advise you to get following tests done:
1. Fasting Blood Sugar- to rule out diabetes
2. Thyroid Profile- to rule out hypothyroidism
3. Hemogram- to check for anemia.
4. Serum Calcium and Vitamin D levels- to see for the deficiencies.
If any issues are found in your reports you would be treated accordingly. Otherwise your Doctor will treat you with SSRI's like Fluoxetine, Escitalopram.

I have actually started taking "Escitalopram" for last 15 days. I had hypertyrodism once, although, it was cured with carbimazole. I check my thyroid every 3 months. I have few other concerns, hope you can help.

1. I am not anxious at all. I fall in sleep in less than 5 minutes while going to bed, and I can almost do it at anytime of the day. So basically I do not have any anxiety or tension. Does that rule out low level of serotonin? Could I be deficient in dopamine?

2. I have been masturbating almost everyday in last 3 years. I am not sure, but it seems to have become an addiction for me. Could this be related to fallen energy level? I find it hard to orgasm while on escitalopram though.

I do not snore according to my family. I almost can go to sleep at anytime I feel tired or can't keep my eyes open of anytime of the day/night.

Hello,
See there is no point in taking Escitalopram with out a doctor's advice. Infact, Escitalopram can also cause excessive sedation in many people. So, that might be the cause, if you are getting the symptoms after the Escitalopram, you need to stop it.
Dopamine deficiency doesn't cause all these issues. Masturbation has no relation to any disease at all, other that Psycological.
I would advice you first of all to get the above mentioned tests done, whichever have not been performed.
It's not that if you are not anxious and you don't have any problem in inducing sleep, then you don't have any depression.
An evaluation is must. Moreover, you had weight loss, that required serious attention.
So, get evaluated and write back to me with reports.

I have submitted the bloods and waiting for the reports. I had few other symptoms.

1. I get extreme fatigue after each bowl movement. I feel muscle weakness. My stools are not like diarrhoea, but they are small in chunk and sticky.

2. Most of the time, I have a psychological feelings of enjoying the hunger. I am not sure why is that. But I enjoy killing appetite.

Let me know if you can find anything noticeable here.

Hi your remaining symptoms are suggestive of Irritable bowel syndrome that is too a functional disorder.
You need to work upon your stress levels and all these symptoms will improve.

Hello,

My CBC report came up. I have high neutrophile (87.2%) and low Lymphocyte (9.1%). Does it indicate anything? Thanks.
I take the following medication for your knowledge:

1. Lexapro 20mg
2. Panax Ginseng 2g
3. Salmeterol 50 ug
2. Flucasone Propionate 500 ug

Let me know if any of them can increase those values. Thanks.

You may carry on with these medications.
Now, you have mentioned here that you use inhalers for respiratory ailments, it means you have chest congestion, that can lead to disturbed sleep and again tiredness the whole day as discussed.
In my opinion you should go for Sleep Study.
